Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the Army Achievement Medal (AAM) (3rd Award). 2. The applicant states he received the additional AAM as an expiration term of service award after he was released from active duty. 3. The applicant provides a DA Form 638 (Recommendation for Award). CONSIDERATION OF EVIDENCE: 1. On 11 March 2009, the applicant enlisted in the Regular Army. 2. He provides a DA Form 638, dated 13 March 2013, showing he was recommended for award of the AAM with 2nd Oak Leaf Cluster for the period 14 December 2011 to 23 October 2012. The form shows the request for award was signed by the approval authority, but not dated. It does not contain a permanent order number and date. 3. On 10 December 2012, he was honorably released from active duty. He completed 3 years and 9 months of creditable active service. His DD Form 214 shows he was awarded the AAM (2nd Award). 4. Army Regulation 600-8-22 (Military Awards) states the AAM is awarded to members of the Armed Forces of the United States, who while serving in a noncombat area on or after 1 August 1981, distinguished themselves by meritorious service or achievement. As with all personal decorations, formal recommendations, approval through the chain of command, and announcement in orders are required. DISCUSSION AND CONCLUSIONS: 1. The applicant’s contention is noted. However, there is insufficient evidence to support his request. 2. The DA Form 638 he provides showing he was recommended for the AAM (3rd Award) is incomplete in that it does not contain an order number and date. Therefore, absent a permanent orders number and date for the award there is an insufficient basis for granting the requested relief.
