The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) as follows: * item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) to show the Noncommissioned Officer (NCO) Professional Development Ribbon with Numeral 3 * item 14 (Military Education) to add the Senior Leadership Course, 6 weeks and 2 days; and the one-week Contracting Officer Representative Course * item 19b (Nearest Relative) to show the correct spelling of his wife's last name 2. The applicant states: * he graduated from the Advanced NCO Course and the Contracting Officer Representative Course but his servicing personnel office did not add the courses to his Enlisted Record Brief * his graduation from the Advanced NCO Course should qualify him for the NCO Professional Development Ribbon with Numeral 3 vice 2nd Award * he listed his wife as the nearest relative in item 19b but her last name is misspelled 3. The applicant provides: * DD Form 214, ending on 31 July 2013 * DA Form 1059 (Service School Academic Evaluation Report) * Certificate of completion of the Contracting Officer Representative Course * Certificate of Training for the Property Book Unit Supply Enhanced Training Module * Certificate of completion of the Unit Supply Specialist Senior Leader Course CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 28 August 1992 and he held military educational specialty 92Y (Unit Supply Specialist). His last name and that of his spouse is listed as "V---odas" on his enlistment document and all other documents throughout his military service. 2. He served through multiple reenlistments in a variety of assignments and he attained the rank/grade of sergeant first class (SFC)/E-7. He retired on 31 July 2013 and he was placed on the Retired List in his retired rank/grade of SFC/E-7. His DD Form 214 shows in: * Item 13, he was awarded or authorized the: * Meritorious Service Medal (2nd Award) * Army Commendation Medal (3rd Award) * Joint Service Achievement Medal * Army Achievement Medal (5th Award) * Meritorious Unit Commendation * USAF Presidential Unit Citation * Army Good Conduct Medal (6th Award) * National Defense Service Medal (3rd Award) * Global War on Terrorism Expeditionary Medal (2nd Award) * Global War on Terrorism Service Medal * Korea Defense Service Medal * NCO Professional Development Ribbon (2nd Award) * Army Service Ribbon * Overseas Service Ribbon (5th Award) * Driver and Mechanic Badge * Item 14, he completed the: * Basic NCO Course, 3 weeks, 1996 * Battle Staff Course, 4 weeks, 2010 * Combatives Level 1, 1 week, 2006 * Force XXI Battle Command Brigade, 1 week 2006 * Inspector General Course, 4 weeks, 1996 * Primary Leadership Development Course, 4 weeks, 1996 * Property Book Unit Supply Enhancement, 1 week, 2007 * Small Arms Maintenance, 2 weeks, 1998 * Standard Property Book Systems Course, 1 week, 1999 * Unit Level Logistics System, S-4 Module Operator/Supervisor Training, 1 week, 1998 * Item 18 (Remarks), entries pertaining to his combat service/deployments: * Service in Iraq, 20030421-20040721 * Service in Djibouti, 20080315-20090609 * Service in Kuwait, 20100710-20101106 * Service in Kuwait, 20110607-20111205 * Item 19b, the name and address of his spouse; her last name is spelled "V---adas" instead of "V---odas" 3. He provides: a. DA Form 1059 and a certificate of training, dated 23 February 2010, that shows he completed the 6-week Unit Supply Specialist Senior Leader Course from 5 January 2010 to 23 February 2010, at Fort Lee, VA. b. Certificate of training, dated 12 February 2010, that shows he completed the Contracting Officer Representative Course (40 hours) at Fort Lee, VA, in what appears to be during attendance at the Senior Leader Course. 4. Army Regulation 600-8-22 (Military Awards): a. The NCO Professional Development Ribbon consists of the basic ribbon with numeral devices of 2, 3, or 4, which signify satisfactory completion of the respective levels of NCO professional development courses. Numerals used in conjunction with this service ribbon are the same type as those used for subsequent awards of the Air Medal. Effective 30 March 1989, a service member will be awarded the NCO Professional Development Ribbon with the numeral which identifies the highest level of NCOES or RC–NCOES successfully completed as follows: 1=Bar Ribbon Device=Primary Level; 2=Basic Level; 3=Advanced Level; and 4=Senior Level. The Advanced level consist of the Advanced NCO Courses (Service School—ANCOC) for award of numeral 3. b. The Iraq Campaign Medal is awarded to members who have served in direct support of Operation Iraqi Freedom (OIF). The area of eligibility encompasses all the land area of the country of Iraq, the contiguous water area out to 12 nautical miles, and all air spaces above the land area of Iraq and above the contiguous water area out to 12 nautical miles. The Iraq Campaign Medal period of eligibility is on or after 19 March 2003 through 31 December 2011. A bronze service star is authorized for wear with this medal for participation in each credited campaign. Approved campaigns are: * Liberation of Iraq (19 March 2003-1 May 2003) * Transition of Iraq (2 May 2003-28 June 2004) * Iraqi Governance (29 June 2004-15 December 2005) * National Resolution (16 December 2005-9 January 2007) * Iraqi Surge (10 January 2007-31 December 2008) * Iraqi Sovereignty (1 January 2009-31 August 2010) * New Dawn (1 September 2010-31 December 2011) c. The Global War on Terrorism Expeditionary Medal is authorized for award to members of the Armed Forces of the United States who deployed abroad for service in Global War on Terrorism operations on or after 11 September 2001 to a date to be determined. The general area of eligibility (AOE) encompasses all foreign land, water, and air spaces outside the fifty states of the United States and outside 200 nautical miles of the shores of the United States in operations approved by the Secretary of Defense. Initial award of the Global War on Terrorism Expeditionary Medal was limited to service members deployed abroad in Operations Enduring Freedom and Iraqi Freedom in designated specific geographic AOE, including Kuwait and Djibouti. Only one award of the Global War on Terrorism Expeditionary Medal may be authorized to any individual; therefore, second or subsequent awards will not be awarded. d. Service members qualified for the Global War on Terrorism Expeditionary Medal by reason of service between 19 March 2003 and 28 February 2005, in an area for which the Iraq Campaign Medal was subsequently authorized, will remain qualified for that medal. Upon application, any such service member may be awarded the Iraq Campaign Medal in lieu of the Global War on Terrorism Expeditionary Medal for such service. No service member will be entitled to both medals for the same act, achievement, or period of service. e. The National Defense Service Medal is awarded for honorable active service for any period between 27 July 1950 and 27 July 1954, 1 January 1961 and 14 August 1974, 2 August 1990 and 30 November 1995, and 11 September 2001 and a date to be determined. Second and subsequent awards of the National Defense Service Medal are denoted by a bronze service star affixed to the National Defense Service Medal. 5. Army Regulation 635-8 (Separation Processing and Documents) prescribes the transition processing function of the military personnel system. This new regulation provides principles of support, standards of service, policies, tasks, rules, and steps governing required actions in the field to support processing personnel for separation and preparation of separation documents. Chapter 5 covers the preparation of the DD Form 214. a. Item 14, list all formal, in-service (full time attendance) training courses successfully completed during the period of service covered by the DD Form 214 of at least 1 week or 40 hours duration. Include course title, length in weeks, and year completed. This information is to assist the Soldier in job placement and counseling; therefore, do not list training courses for combat skills. Acceptable source documents include the enlisted record brief, DA Form 4037 (Officer Record Brief), DA Form 1059, or other military issued certificate of completion with from and through dates or number of weeks. b. Item 19b, the data source is as provided by the Soldier. Advise the Soldier that the name and address of a relative should be someone who will know Soldier’s location and address at all times. DISCUSSION AND CONCLUSIONS: 1. The applicant completed the Senior Leader Course from 5 January 2010 to 23 February 2010. This entitles him to award of the NCO Professional Development Ribbon with Numeral 3 vice the current entry of "2nd Award," and correction of his DD Form 214 to show this award. 2. The applicant served in Iraq from 20030421 to 20040721, Djibouti from 20080315 to 20090609, and twice in Kuwait from 20100710 to 20101106 and from 20110607 to 20111205. a. He served a qualifying period of service for award of the Iraq Campaign Medal (20030421-20040721) and he participated in three campaigns during this period. Therefore, he is entitled to the Iraq Campaign Medal with three bronze service stars and correction of his D Form 214 to show this award. b. He is entitled to award of the Global War on Terrorism Expeditionary Medal based on his service in Kuwait. His DD Form 214 listed the Global War on Terrorism Expeditionary Medal (2nd Award). Only one award of the Global War on Terrorism Expeditionary Medal is authorized. However, it is the policy of this Board not to correct an administrative error if the correction would result in a less favorable action to the applicant. The Board will not change this entry without his specific request. c. He served during two periods of service - not three - for award of the National Defense Service Medal. His DD Form 214 listed the National Defense Service Medal (3rd Award), an erroneous entry that may only be corrected with his specific request due to the Board's policy not to correct an administrative error if the correction would result in a less favorable action to the applicant. 3. He completed the Senior Leader Course from 5 January 2010 to 23 February 2010. During attendance at this course, he also completed the Contracting Officer Representative Course on 12 February 2010. Only one course completion is authorized when two courses fall during the same period. As a result, he is entitled to correction of item 14 of his DD Form 214 to show the Senior Leader Course but not the Contracting Officer Representative Course. 4. His wife's name is misspelled in item 19b. It should be corrected to show her last name as "V---odas" instead of "V---adas." The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by: * deleting from item 13 the NCO Professional Development Ribbon (2nd Award) and adding the NCO Professional Development Ribbon with Numeral 3 and the Iraq Campaign Medal with three bronze service stars * adding to item 14 the Senior Leader Course, 6 weeks, 2010 * deleting from item 19b the current last name of his spouse and adding the same last name as his 2. The Board further determined that the evidence presented was insufficient to warrant a portion of the requested relief.
