THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the Army Commendation Medal and the Combat Action Badge (CAB). 2. He states he was not awarded the CAB even though he met the requirements for the award. 3. He provides his DD Form 214, DA Form 638 (Recommendation for Award), and a DA Form 2823 (Sworn Statement). CONSIDERATION OF EVIDENCE: 1. The applicant is requesting correction of his records to show award of the CAB. The U.S. Army Human Resources Command (HRC) provided processing procedures for award of the CAB. Therefore, he must submit his request for the award of the CAB to HRC for review and possible administrative correction. He has been notified by separate correspondence of the procedures for applying for this badge. As a result, the CAB will not be discussed further in this Record of Proceedings. 2. After having prior service in the U.S. Navy and the Army National Guard, on 27 February 2008, the applicant enlisted in the Regular Army. He served in Iraq from 3 August 2009 to 26 July 2010. On 3 November 2011, he was discharged under the provisions of Army Regulation 635-200 (Active Duty Enlisted Administrative Separations), by reason of misconduct with a characterization of service of general, under honorable conditions. 3. His DD Form 214 does not show award of the ARCOM. 4. The applicant provided a DA Form 638, dated 8 May 2010, that shows he was awarded the ARCOM by Permanent Orders Number 137-101, dated 23 June 2010, issued by Headquarters, 3-2 Stryker Brigade Combat Team, Forward Operation Base, Warhorse, Iraq. A copy of the DA Form 638 or the certificate is not contained in his records. DISCUSSION AND CONCLUSIONS: Permanent orders verify that the applicant was awarded the ARCOM. Therefore, his DD Form 214 should be corrected to show this award.
