THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests the Bronze Star Medal (BSM) and two additional awards of the Army Achievement Medal (AAM) be added to his DD Form 214 (Certificate of Release or Discharge from Active Duty). He also requests his DD Form 214 be corrected to show he completed Sniper School. 2. The applicant states: * he received orders for the AAM * the Sniper School is not listed on his DD Form 214 3. The applicant provides: * orders for four awards of the AAM * orders for the BSM * Enlisted Record Brief (ERB) * DD Form 214 CONSIDERATION OF EVIDENCE: 1. Having prior active service in the Regular Army (RA), the applicant enlisted in the RA on 29 November 2005 and trained as an infantryman. He served three tours in Iraq and one tour in Afghanistan. 2. Permanent Order 222-01, undated, awarded him the AAM for meritorious service during the period 1 April 2002 to 17 October 2004. 3. Permanent Order 007-04, dated 7 January 2009, awarded him the AAM for outstanding participation in the Army Physical Fitness Test on 7 October 2008. 4. Permanent Order 159-404G, dated 8 June 2009, awarded him the BSM for meritorious service during the period 20 October 2008 to 20 October 2009. 5. Permanent Order 147-20, dated 27 May 2011, awarded him the AAM for meritorious achievement during the period 11 April 2011 to 18 April 2011. 6. Permanent Order 340-002, dated 6 December 2011, awarded him the AAM for outstanding achievement during the period 10 October 2011 to 9 December 2011. 7. On 22 July 2013, he was discharged. 8. His DD Form 214 shows he was awarded or authorized the: * Army Commendation Medal (3rd Award) * AAM (2nd Award) * Meritorious Unit Commendation * Army Good Conduct Medal (2nd Award) * National Defense Service Medal * Global War on Terrorism Expeditionary Medal * Global War on Terrorism Service Medal * Iraq Campaign Medal with three bronze service stars * Army Service Ribbon * Overseas Service Ribbon (4th Award) * Combat Infantryman Badge * Expert Infantryman Badge * Ranger Tab * Parachutist Badge 9. Item 14 (Military Education) of his DD Form 214 does not show he completed Sniper School. 10. There is no evidence of record and the applicant did not provide any evidence that shows he completed Sniper School. 11. Section VI (Military Education) of his ERB does not show he completed Sniper School. 12. Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214. It states for item 14, list formal in-service (full-time attendance) training courses of at 40 hours successfully completed (include title, length in weeks, and month and year completed) – this information is to assist the Soldier in job placement and counseling; therefore, do not list training courses for combat skills. DISCUSSION AND CONCLUSIONS: 1. The applicant's DD Form 214 shows two awards of the AAM, but orders show he received four awards of the AAM. Therefore, his DD Form 214 should be corrected to show two additional awards of the AAM. 2. Orders show he received the BSM. Therefore, his DD Form 214 should be corrected to show this medal. 3. There is no evidence of record and he provided no evidence that shows he completed Sniper School. Therefore, there is no basis for granting this portion of his request. The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by: a. deleting the entry "ARMY ACHIEVEMENT MEDAL (2ND AWARD)" from his DD Form 214 for the period ending 22 July 2013; and b. adding the Army Achievement Medal (4th Award) and Bronze Star Medal to his DD Form 214 for the period ending 22 July 2013. 2. The Board further determined that the evidence presented was insufficient to warrant a portion of the requested relief.
