THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests that the DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) of her deceased husband, a former service member (FSM), be corrected to show the first letter of his first name is a “T” instead of a “J.” 2. The applicant states, in effect that the FSM’s first name was spelled incorrectly on his DD Form 214. 3. The applicant provides copies of the FSM’s death certificate and their marriage license. CONSIDERATION OF EVIDENCE: 1. On 5 April 1967, the FSM enlisted in the Regular Army and a statement of personal history, which was prepared at the time of enlistment, lists the first letter of his first name as "T." 2. He was awarded military occupational specialty 76S (automobile repair parts specialist) and he served in Vietnam from 11 January 1968 to 8 January 1969. 3. On 3 April 1970, he was honorably released from active duty (REFRAD due to the expiration of his term of service. He had served 2 years, 11 months, and 29 days of active service and the DD Form 214 issued at the time of his REFRAD shows the first letter of his first name as “J” instead of “T.” 4. On 1 April 1973, he was honorably discharged from the U.S. Army Reserve (USAR) and the discharge order he was issued shows the first letter of his first name was a "T." DISCUSSION AND CONCLUSIONS: 1. The applicant’s contention that the FSM's first name is misspelled has been noted and found to have merit. 2. The FSM enlisted with a first name beginning with the letter “T” and served his entire period of service using that first name. However, it is apparent that an administrative error occurred at the time of his REFRAD and his first name was misspelled with a “J” instead of a “T.” 3. Accordingly, his DD Form 214 should be corrected to show the spelling of his first name is the same as it is reflected on his discharge orders from the USAR and his death certificate.
