THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests removal of a North Atlantic Treaty Organization (NATO) Certificate from her Army Military Human Resource Record (AMHRR). 2. The applicant states she has two NATO Certificates that were placed in her AMHRR, but the dates are different. The correct certificate is for the period 2 May 2012 to 26 January 2013 and the certificate to be removed is for the period 28 April 2012 to 22 January 2013. 3. She further states she was assigned as the S-1 for the 8th Squadron, 1st Cavalry Regiment, Joint Base Lewis-McChord, WA. Before her permanent change of station, her NATO certificate with the correct period was submitted for filing in her AMHRR. After she departed, a second NATO Certificate was uploaded to her file by the new S-1 with the inaccurate dates. The S-1 tried to reconcile the records and unknowingly uploaded a second certificate. 4. The applicant provides two NATO Certificates. CONSIDERATION OF EVIDENCE: 1. The applicant was appointment as a Reserve commissioned officer on 7 May 2009. She was promoted to captain on 1 December 2012. 2. The Performance folder of her AMHRR contains two NATO Certificates for the periods 2 May 2012 to 26 January 2013 and 28 April 2012 to 22 January 2013. Both certificates show she was awarded the NATO Medal in relation to the International Security Assistance Force (ISAF) Operations in Afghanistan. 3. The Defense Finance and Accounting Service (DFAS) confirmed the applicant received hostile fire/imminent danger pay (HF/IDP) and combat zone tax exclusion (CZTE) during the period 30 April 2012 through 23 January 2013 for service in Afghanistan. 4. Army Regulation 600-8-104 (AMHRR Management) prescribes the policies governing the AMHRR and Army Personnel Qualification Records. Paragraph 2-4 states that once a document is placed in the AMHRR, it becomes a permanent part of that file and will not be removed from that file or moved to another part of the file unless directed by, among other agencies, the Army Board for Correction of Military Records. DISCUSSION AND CONCLUSIONS: 1. The evidence of record shows two NATO Certificates for approximately the same period of service and for the same operation that were filed in the applicant's AMHRR. Although DFAS verified she received HF/IDP and CZTE during the period 30 April 2012 through 23 January 2013, based on her statement it appears the NATO Certificate for the period 2 May 2012 to 26 January 2013 is the appropriate certificate that should remain in her AHMRR. 2. In view of the foregoing, the applicant's records should be corrected as recommended below.
