THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 31 January 2014 to show in: * item 7a (Place of Entry into Active Duty), "Fort Benning, Georgia" * item 7b (Home of Record (HOR) at Time of Entry), "XXXX Maracaibo Drive, Lake Havasu City, Arizona 86403" 2. The applicant states items 7a and 7b of his enlisted service DD Form 214 are correct. In 1993, he enlisted for the Officer Candidate School (OCS) Enlistment Option Program. He entered active duty at the Military Entrance Processing Station (MEPS) station in Phoenix, AZ. His HOR was "XXXX Maracaibo Drive, Lake Havasu City, AZ." Upon graduation from OCS at Fort Benning, GA, he was discharged from enlisted service and immediately returned to active duty as a commissioned officer. Therefore, his final DD Form 214 should reflect his place of entry into active duty as Fort Benning, GA. 3. The applicant provides: * DD Form 4 (Enlistment/Reenlistment Document), dated 24 May 1993 * Headquarters, U.S. Army Infantry Center Orders 61-85 * Headquarters, U.S. Army Infantry Center Orders 19-2-A-113 * DD Form 214 for the period ending 13 April 1994 * Commissioned Officer Appointment Memorandum, dated 14 April 1994 * DD Form 214 for the period ending 31 January 2014 CONSIDERATION OF EVIDENCE: 1. On 5 October 1993, the applicant enlisted in the Regular Army for the U.S. Army Officer Enlistment Program. 2. His DD Form 4 shows his HOR as "XXXX Maracaibo Drive, Lake Havasu City, AZ 86403" and his place of enlistment as Phoenix MEPS, Phoenix, AZ. 3. On 13 April 1994, he was discharged as an enlisted Soldier for the purpose of accepting a commission in the U.S. Army Reserve. The DD Form 214 he was issued for this period of service shows he was separated at Fort Benning, GA. 4. On 14 April 1994, he was appointed as a Reserve Commissioned Officer in the rank of second lieutenant. 5. Headquarters, U.S. Army Infantry Center Orders 19-2-A-113, dated 24 March 1994, show his place of entry on active duty as a commissioned officer as Fort Benning, GA. 6. On 31 January 2014, he was retired from active duty with sufficient service for retirement in the rank of lieutenant colonel. The DD Form 214 he was issued for this period of service shows in: * item 7a, "Fort Knox, Kentucky" * item 7b, "XXXX Maracaibo Dr, Phoenix, Arizona 86403" 7. Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214. It states a Soldier’s initial enlistment contract or appointment document is the source for the data for items 7a and 7b. It states to enter the city and state where the Soldier entered active duty in item 7a. It further states that the "Home of Record" is the place recorded as the HOR of the Soldier when commissioned, appointed, enlisted, or ordered to a tour of active duty. DISCUSSION AND CONCLUSIONS: 1. The applicant's HOR city upon his initial enlistment was reflected as "Lake Havasu City." His DD Form 214 for the period ending 31 January 2014 shows his HOR city as "Phoenix." As such, it appears "Phoenix" was entered on this form in error. Therefore, he is entitled to correction of this DD Form 214 to show his HOR as "XXXX Maracaibo Drive, Lake Havasu City, AZ 86403." 2. Evidence shows he was ordered to active duty as a commissioned officer at Fort Benning, GA. Therefore, he is entitled to correction of his final DD Form 214 to show his place of entry on active duty as Fort Benning, GA. 3. In view of the foregoing, the applicant is entitled to have his records corrected as shown below.
