Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of item 12b (Separation Date This Period) of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show she was separated on 10 November 2011 instead of 11 October 2011. 2. The applicant states a clerical error occurred at the time of her release from active duty (REFRAD) that reversed the day and month of her REFRAD. 3. The applicant provides copies of her DD Form 214, separation orders, email, and a letter from her Congressional representative. CONSIDERATION OF EVIDENCE: 1. The applicant was serving as a U.S. Army Reserve lieutenant colonel when she was ordered to active duty on 22 April 2011 in support of Operation New Dawn in Iraq. 2. She deployed to Iraq during the period 8 June 2011 to 1 November 2011 and returned to Fort Dix, New Jersey, where she was honorably REFRAD on 10 November 2011. However, item 12b of her DD Form 214 was incorrectly prepared to show she was REFRAD on 11 October 2011. She completed 6 months and 19 days of active service during this period instead of the 5 months and 20 days reflected in item 12c (Net Active Service This Period) of her DD Form 214. DISCUSSION AND CONCLUSIONS: 1. The applicant was REFRAD on 10 November 2011; however, a clerical error occurred at the time her DD Form 214 was prepared indicating she was REFRAD on 11 October 2011. 2. Accordingly, her DD Form 214 should be corrected to show she was REFRAD on 10 November 2011 and completed 6 months and 19 days of active service during this period.
