THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show an additional award of the Army Commendation Medal. 2. The applicant states the award was given to her after her discharge date. 3. The applicant provides: * self-authored statement * letter from the Army Review Boards Agency * DA Form 4980-14 (Army Commendation Medal Certificate) * DA Form 638 (Recommendation for Award) CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 15 April 2003. She was honorably discharged on 16 July 2013 after completing 10 years, 3 months, and 2 days of active service. 2. Her DD Form 214 shows she served in Iraq from 28 November 2004 to 12 November 2005 and in Afghanistan from 21 May 2011 to 19 May 2012. 3. Her DD Form 214 shows she was awarded or authorized the: * Army Commendation Medal (3rd Award) * Army Achievement Medal (3rd Award) * Meritorious Unit Commendation * Army Superior Unit Award * Army Good Conduct Medal (3rd Award) * National Defense Service Medal * Afghanistan Campaign Medal with two campaign stars * Global War on Terrorism Service Medal * Iraq Campaign Medal with one campaign star * Noncommissioned Officer Professional Development Ribbon (2nd Award) * Army Service Ribbon * Overseas Service Ribbon (2nd Award) * North Atlantic Treaty Organization Medal 4. The applicant provided a copy of Headquarters, 2nd Brigade Combat Team, 4th Infantry Division, Fort Carson, CO, Permanent Order Number 234-009, dated 28 August 2013, which shows she was awarded the Army Commendation Medal with 3rd Oak Leaf Cluster (indicating the fourth award) for exceptionally meritorious service and performance while serving as the unit supply noncommissioned officer during the period 24 January 2011 to 17 July 2013. DISCUSSION AND CONCLUSIONS: The evidence clearly shows a fourth Army Commendation Medal was awarded to the applicant after her discharge; therefore, it was not included on her DD Form 214. In light of the foregoing, her DD Form 214 should be corrected to show this award.
