Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to list the Combat Infantryman Badge (CIB) and the Army Commendation Medal. 2. The applicant states he received these two awards and they are not listed on his DD Form 214. Both of these awards were received after the paperwork for processing his DD Form 214 was submitted. 3. The applicant provides: * Permanent Orders Number 21-17 * DD Form 214 * DA Form 638 (Recommendation for Award) * Army Commendation Medal certificate CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 17 January 2008. He served as a special forces medical sergeant. 2. He provided a copy of Permanent Orders 21-17 awarding him the CIB for actively engaging or being engaged by the enemy on 22 November 2012. 3. He was honorably released from active duty in pay grade E-6 on 16 July 2013 His DD Form 214 lists in: * Item 13 (Decorations, Medal, Badges, Citations, and Campaigns Ribbons Awarded or Authorized) the: * Army Good Conduct Medal * National Defense Service Medal * Global War on Terrorism Service Medal * Army Service Ribbon * Combat Infantryman Badge * Special Forces Tab * Parachutist Badge * Item 18 (Remark): Service in Afghanistan from 20120910-20130225 4. The applicant requested that the CIB be added to his DD Form 214. His DD Form 214 currently shows he was awarded the CIB Therefore, this portion of the applicant's 's request will not be discussed further in these proceedings. 5. He also provided copies of: * a DA Form 638, dated 4 September 2013, recommending him for award of the Army Commendation Medal for exceptionally meritorious service for the period of service from 17 January 2008 through 17 July 2013; the award was approved on 14 October 2013 and issued by Permanent Orders Number 262-02 * Certificate, dated 19 September 2013, awarding him the Army Commendation Medal for exceptionally meritorious service while serving as a special forces medical sergeant from 17 January 2008 through 17 July 2013, issued by Permanent Orders Number 262-02 6. Army Regulation 600-8-22 (Military Awards) states the Afghanistan Campaign Medal is awarded to members who served in direct support of OEF. The area of eligibility encompasses all the land area of the country of Afghanistan and all air spaces above the land area. The period of eligibility is on or after 24 October 2001 to a future date to be determined by the Secretary of Defense or the cessation of OEF. A bronze service star is authorized for wear with this medal for participation in each credited campaign. During the applicant's service in Afghanistan he participated in the Consolidation I (1 December 2001-30 September 2006) campaign. DISCUSSION AND CONCLUSIONS: 1. Permanent Orders awarded the applicant the Army Commendation Medal on 14 October 2013. The orders were issued after his release from active duty on 16 July 2013. Therefore, he is entitled to correction of his DD Form 214 to show this award. 2. The applicant served in Afghanistan during a qualifying period of service for the Afghanistan Campaign Medal and he participated during one campaign phase. Therefore, he is entitled to award of the Afghanistan Campaign Medal with one bronze service star and correction of his DD Form 214 to show this award.
