Application for correction of military records (with supporting documents provided, if any). 2. Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests reimbursement of $305.17 that was collected from her military pay as a result of the loss of Table of Allowance (TA)-50 organizational equipment. 2. The applicant states the equipment was subsequently found and turned in to the Central Issue Facility on 9 September 2013. She received a DA Form 3645 (Organizational Clothing and Individual Equipment Record). Prior to her retirement, she was unable to find the equipment. Accordingly, a statement of charges was initiated against her and the amount of $305.17 was deducted from her pay. However, she subsequently found the equipment and turned the equipment in to CIF on 13 September 2013. Therefore, she is rightfully entitled to reimbursement of the lost items. 3. The applicant provides: * DA Form 362 (Statement of Charges/cash Collection Voucher) * DA Form 3645 (Clothing Transaction Record - Automated) CONSIDERATION OF EVIDENCE: 1. The applicant was appointed as a commissioned officer on 29 July 1988. She entered active duty on 7 March 1989. She served in a variety of assignments and attained the rank of lieutenant colonel (LTC). 2. On 6 December 2012, her commander initiated and submitted to the servicing finance office a DD Form 362 in the amount of $305.17 for the loss of several organizational items/equipment. 3. She retired on 31 December 2012 and she was placed on the Retired List in her retired rank of LTC on 1 January 2013. 4. She provides an automated DA Form 3645, dated 13 September 2013, confirming she returned the lost items to the Fort Sam Houston, TX CIF. 5. A staff member of the Board confirmed via phone call on 9 July 2014 with an official at the Fort Sam Houston, TX CIF the return of all equipment. 6. in the processing of this case an advisory opinion was received from the Office of the Deputy Chief of Staff, G-4 on 20 February 2013. The advisory official recommended approval. He states that after a review of the available documents, the applicant recovered the missing government equipment and turned it in to the Fort Sam Houston CIF. 7. The applicant was provided a copy of this advisory opinion for information and to allow her the opportunity to submit comments or a rebuttal, but she did not respond. DISCUSSION AND CONCLUSIONS: Upon her retirement processing in 2012, the applicant appears to have misplaced certain government-issued items/organizational equipment. As a result, her chain of command initiated a statement of charges/cash collection voucher in the amount of $305.17 against her. After her retirement, she was able to locate the missing items and turn them in to the appropriate CIF. Therefore, she is entitled to reimbursement of this amount.
