THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant, the daughter of a deceased former service member (FSM), requests correction of the FSM's DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) to show all awards the FSM is authorized as a result of his active duty service. 2. The applicant states the FSM was in the service for 3 years and he was awarded several medals for his service in Vietnam. 3. The applicant provides a DD Form 214 and a death certificate. CONSIDERATION OF EVIDENCE: 1. The FSM was inducted into the Army of the United States on 14 October 1963 and he was awarded military occupational specialty 94B (Cook) upon completion of initial entry training. 2. Special Orders Number 203, dated 20 November 1963, issued by the U.S. Army Training Center, Fort Ord, CA awarded him the Marksman Marksmanship Qualification Badge with Rifle Bar. 3. The FSM's DA Form 24 (Service Record) shows he served in Alaska from 7 January 1964 to 17 July 1965. It also shows he received all "excellent" conduct and efficiency ratings throughout his active duty service. 4. The FSM's record is void of derogatory information or a unit commander's disqualification that would have precluded him from being awarded the Army Good Conduct Medal (AGCM). 5. The FSM's DD Form 214 shows he was honorably released from active duty on 21 July 1965 in the rank of specialist four after completing 1 year, 9 months, and 8 days of active service. The DD Form 214 does not show any authorized awards. 6. There is no evidence in the FSM's available records of service in Vietnam. 7. Army Regulation 600-8-22 (Military Awards) states the National Defense Service Medal is awarded for honorable active service for any period between 27 July 1950 and 27 July 1954, 1 January 1961 and 14 August 1974, 2 August 1990 and 30 November 1995, and 11 September 2001 and a date to be determined. 8. Army Regulation 672-5-1 (Awards), in effect at the time, stated the AGCM was awarded for each 3 years of continuous enlisted active Federal military service completed on or after 27 August 1940; for the first award only, 1 year served entirely during the period 7 December 1941 to 2 March 1946; and, for the first award only, upon termination of service on or after 27 June 1950 of less than 3 years but more than 1 year. The enlisted person must have had all "excellent" conduct and efficiency ratings. DISCUSSION AND CONCLUSIONS: 1. The FSM served a period of qualifying honorable service for award of the National Defense Service Medal. Therefore, this award should be added to his DD Form 214. 2. The evidence of record shows the FSM received all "excellent" conduct and efficiency ratings throughout his active duty service and his records are void of derogatory information or a commander's disqualification that would have precluded him from being awarded the AGCM. Therefore, it would be appropriate to award him the first award of the AGCM for the period 14 October 1963 to 21 July 1965 and to add this award to his DD Form 214. 3. Special orders show the FSM earned the Marksman Marksmanship Qualification Badge with Rifle Bar. This badge should also be added to his DD Form 214.
