THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the: * Joint Meritorious Unit Award * Army Reserve Components Overseas Training Ribbon 2. The applicant states both awards are not listed on her DD Form 214 for the period ending 18 September 2012. 3. The applicant provides the following documents: * DD Form 214 for the period ending 18 September 2012 * Joint Meritorious Unit Award Citation * Joint Meritorious Unit Award Order * Memorandum, subject, Army Reserve Components Overseas Training Ribbon CONSIDERATION OF EVIDENCE: 1. Having had prior enlisted service, the applicant was appointed as Reserve commissioned officer of the Army, Medical Services Corps, and executed an oath of office on 3 December 1998. 2. On 8 September 2004, the U.S. Army Human Resources Command issued her a Notification of Eligibility for Retired Pay at Age 60 (20-Year Letter). 3. She entered active duty on 30 September 2005 in support of Operation Enduring Freedom. She was honorably released from active duty on 28 September 2009. 4. She again entered active duty on 17 July 2011 and served at Guantanamo Bay, Cuba, from 9 August 2011 to 29 July 2012. She was assigned to Joint Task Force Guantanamo. She was honorably released from active duty on 18 September 2012. Her DD Form 214 does not show the requested awards. 5. She provides: a. Joint Staff Permanent Order Number J-1SO-00239-13 and a Citation, issued by the Joint Staff, awarding Joint Task Force Guantanamo the Joint Meritorious Unit Award for service from 16 June 2011 to 30 June 2013. b. A memorandum, issued by the 416th Army Field Support Brigade Commander, on 29 August 2014, awarding her the Army Reserve Components Overseas Training Ribbon for support of Ulchi Freedom Guardian in Korea from 12 August 2014 to 28 August 2014. DISCUSSION AND CONCLUSIONS: 1. PO awarded the applicant's unit the Joint Meritorious Unit Award which is not shown on her DD Form 214. 2. The applicant served on active duty from 17 July 2011 to 18 September 2012. Her award of the Army Reserve Components Overseas Training Ribbon for support of Ulchi Freedom Guardian in Korea from 12 August 2014 to 28 August 2014 occurred after her release from active duty and for an action that also occurred after her release from active duty. There is no provision to list a future award for an action that had not occurred on a previously-issued DD Form 214. The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding to her DD Form 214 for the period ending 18 September 2012 the Joint Meritorious Unit Award. 2. The Board further determined that the evidence presented is insufficient to warrant a portion of the requested relief.
