THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show his foreign/combat service in support of Operation Iraqi Freedom (OIF). 2. The applicant states his wartime/overseas service is missing from his DD Form 214. He was assigned to the U.S. Forces-Iraq from January to December 2010. 3. The applicant provides: * DD Form 214 * Temporary change of station (TCS) orders * Permanent Orders (PO) 291-036 CONSIDERATION OF EVIDENCE: 1. Having had prior service in the Regular Army (April 1983 to May 1987), he enlisted in the U.S. Army Reserve (USAR) on 15 August 1988. He held military occupational specialty 11B (Infantryman). 2. He served through multiple reenlistments in the USAR. He entered active duty in the Active Guard Reserve (AGR) on 24 January 1999. He served in a variety of stateside or overseas assignments and he was promoted to sergeant first class (SFC)/E-7. 3. On 30 October 2002, the U.S. Army Reserve Personnel Command, St. Louis issued him a Notification of Eligibility for Retired Pay at Age 60 (20-Year Letter). 4. On 16 November 2009, Headquarters, 75th Division, Houston, TX published orders ordering him deployed in TCS status in support of OIF in Baghdad, Iraq, for a period not to exceed 390 days with a proceed date of on or about 8 January 2009 (i.e., 2010). The exact date he deployed is unknown. However, his records contain: a. PO 187-079, issued by Headquarters, U.S. Forces-Iraq, Baghdad, Iraq on 6 June 2010, awarding him the Joint Service Achievement Medal for service from 9 May 2010 to 24 June 2010. b. PO 291-036, issued by Headquarters, U.S. Forces-Iraq, Baghdad, Iraq on 18 October 2010, awarding him the Joint Service Commendation Medal for service from 16 January 2010 to 15 December 2010. c. PO 299-040, issued by Headquarters, U.S. Forces-Iraq, Baghdad, Iraq on 26 October 2010, awarding him the Joint Service Achievement Medal for service from 29 August 2010 to 1 September 2010. d. Noncommissioned Officer Evaluation Report (NCOER) for the rating period 20100116 through 20100715 (6 months) for his duties as Assistant Operations NCO In Charge, U.S. Forces-Iraq, Al Faw Palace, Baghdad e. NCOER for the rating period 20100716 through 20101115 (4 months) for his duties as Joint Operations NCO In Charge, U.S. Forces-Iraq, Consolidates Operating Site (COS) Erbil. 5. He retired on 31 May 2014 by reason of having sufficient service for retirement and he was placed on the Retired List in his retired rank/grade of SFC/E-7. He completed 15 years, 4 months, and 7 days of creditable military service during this period. His DD Form 214 shows in: * Item 12f (Foreign Service) - "0000-00-00" * Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) the: * Armed Forces Reserve Medal with 20-Year Device (Silver Hourglass) * Iraq Campaign Medal with two bronze service stars * Meritorious Service Medal (3rd Award) * Joint Service Commendation Medal * Army Commendation Medal (4th Award) * Joint Service Achievement Medal (2nd Award) * Army Achievement Medal (8th Award) * Army Good Conduct Medal (5th Award) * Army Reserve Component Achievement Medal * National Defense Service Medal (2nd Award) * Global War on Terrorism Service Medal * NCO Professional Development Ribbon with Numeral 3 * Army Service Ribbon * Overseas Service Ribbon * Army Reserve Component Overseas Training Ribbon * Drivers and Mechanic Badge with Driver-Wheeled Vehicle * Item 18 (Remarks) does not show service in a hazardous fire/imminent danger pay (HF/IDP) area 6. Joint Staff PO Number J-1SO-0203-11, dated 21 September 2011, awarded the Joint Meritorious Unit Commendation to Headquarters, U.S. Forces-Iraq for meritorious service from 1 January 2010 to 31 December 2010. 7. Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214. Chapter 2 contains guidance on the preparation of the DD Form 214. It states for: * block 12f enter the total amount of foreign service completed during the period covered by the DD Form 214 * block 13, enter all awards and decorations * block 18 for an active duty Soldier deployed with his or her unit during their continuous period of active service, the statement "Service in (Name of Country Deployed) From YYYYMMDD - YYYYMMDD)" DISCUSSION AND CONCLUSIONS: 1. The available evidence shows the applicant served in Iraq from 16 January 2010 to 15 December 2010, a period of 11 months, as shown on his award of the Joint Service Commendation Medal and supported by his NCOERs for the rating periods for 20100116 through 20100715 (6 months) and 20100716 through 20101115 (4 months). His DD Form 214 currently does not reflect his foreign service or deployment. 2. Joint PO awarded his unit (Headquarters, U.S. Forces-Iraq) the Joint Meritorious Unit Commendation during his assignment to this unit. This joint award is not listed on his DD Form 214.
