THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 28 February 2012 to show: * award of the Joint Chiefs of Staff Identification Badge vice Army Staff Identification Badge * all her military education she completed from 1984 to 2007 2. The applicant states: a. The information on her final DD Form 214 is incorrect or incomplete. There is confusion about her Joint Chiefs of Staff Identification Badge and it is listed incorrectly as the Army Staff Identification Badge. She never worked for the Army Staff but she did work for 6 years on the Joint Staff and has the identification badge. b. Her final DD Form 214 does not have any entries for her military education. She completed the Military Intelligence (MI) Officer Basic Course (OBC) in 1984, Defense Intelligence College Master of Science of Strategic Intelligence (MSSI) in 1988, MI Officer Advance Course (OAC) in 1993, Combined Arms and Services Staff School (CAS3) in 1995, U.S. Marine Corps Command and Staff College (CGSC) in 1997, and the U.S. Army War College (AWC) in 2007. 3. The applicant provides her DD Form 214, DA Form 1059 (Service School Academic Evaluation Report (AER)), and seven certificates, dated between 13 December 1984 and 5 October 2002. CONSIDERATION OF EVIDENCE: 1. The applicant’s records show she was appointed as a Reserve second lieutenant in the U.S. Army Reserve on 1 June 1984. She attended and successfully completed the MI OBC in 1984, MI OAC in 1993, CAS3 in 1995 and the Marine Corps CGSC in 1997. 2. Having had prior active service, she entered active duty as a member of the USAR on 1 October 2001 and was assigned to the Defense Intelligence Agency (DIA) Joint Staff, Washington, DC. She provides a certificate, dated 5 October 2002, issued by the Chairman, Joint Chiefs of Staff, stating she was authorized to wear the Joint Chiefs of Staff Identification Badge. 3. She was honorably released from active duty on 27 March 2004 to the control of the USAR. Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized (All periods of service) of the DD Form 214 she was issued for this period of service shows, in part, she was authorized the Joint Chiefs of Staff Identification Badge. 4. She served on active duty as a member of the USAR from 28 March 2004 to 22 September 2004. The DD Form 214 she was issued for this period of service shows, in part, she was authorized the Army Staff Identification Badge; it does not show the Joint Chiefs of Staff Identification Badge. 5. She served on active duty as a member of the USAR from 1 October 2004 to 27 July 2006. The DD Form 214 she was issued for this period of service shows, in part, she was authorized the Army Staff Identification Badge; it does not show the Joint Chiefs of Staff Identification Badge. 6. She entered active duty as a member of the USAR on 28 July 2006. She attended and successfully completed the AWC from 11 August 2006 to 9 June 2007. She was honorably released from active duty on 15 June 2007 to the control of the USAR. Item 14 (Military Education) of the DD Form 214 she was issued for this period of service contains the entry "United States Army War College, 44 weeks, 2007." Item 18 (Remarks) of this DD Form 214 contains a continuation of her awards from item 13; however, this section is illegible. 7. She entered active duty as a member of the USAR on 2 March 2009 and she was honorably released from active duty on 28 February 2012 to the control of the USAR. The DD Form 214 she was issued for this period of service shows, in part, she was authorized the Army Staff Identification Badge; it does not show the Joint Chiefs of Staff Identification Badge. Item 14 of this DD Form 214 contains the entry "none." 8. On 9 January 2014, she was honorably released from the USAR and transferred to the Retired Reserve. 9. Army Regulation 635-5 (Separation Documents), in effect at the time, prescribed the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. The regulation stated for item 14, list the formal in-service (full-time attendance) training courses successfully completed during the period covered by the DD Form 214. DISCUSSION AND CONCLUSIONS: 1. The evidence of record confirms the applicant was authorized the Joint Chiefs of Staff Identification Badge on 5 October 2002 and it was properly shown on her DD Form 214 for the period ending 27 March 2004. It appears an error occurred during the preparation of her DD Form 214 for the period ending 27 July 2006 and this badge was mistakenly listed as the Army Staff Identification Badge. This error was compounded when her DD Form 214 for the period ending 28 February 2012 was completed listing the Army Staff Identification Badge vice the Joint Chiefs of Staff Identification Badge. She meets the criteria to have this DD Form 214 corrected accordingly. 2. With respect to showing all of her military education on her final DD Form 214, only courses that were successfully completed during the period covered by the DD Form 214 are listed on the DD Form 214. Item 14 of her DD Form 214 for the period ending 28 February 2012 correctly shows the entry "none." The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending her DD Form 214 for the period ending 28 February 2012 by deleting the Army Staff Identification Badge and replacing it with the Joint Chiefs of Staff Identification Badge. 2. The Board further determined that the evidence presented was insufficient to warrant a portion of the requested relief.
