THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of item 12f (Foreign Service) of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show he served in Iraq from January 2011 to February 2012. 2. The applicant states his Iraq service was omitted from his DD Form 214. 3. The applicant provides a copy of his DD Form 214. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 26 October 2009 and he held military occupational specialty 11B (infantryman). 2. He was honorably released from active duty on 28 February 2013 and was transferred to the U.S. Army Reserve. He was credited with completing 3 years, 4 months, and 5 days of active service. His DD Form 214 lists in: * Item 13 (Decorations, Medals, Badges, Citations, and Campaign Ribbons Awarded or Authorized) the: * Army Commendation Medal * National Defense Service Medal * Global War on Terrorism Service Medal * Army Service Ribbon * Driver and Mechanic Badge with Driver-Wheeled Vehicle(s) Clasp * Item 12f – zero credit * Item 18 (Remarks) – No service in Iraq 3. On 30 March 2015, a staff member of the Defense Finance and Accounting Service (DFAS) verified the applicant received hostile fire and imminent danger pay for service in Kuwait from 5 February through 13 December 2011, a period of 10 months and 8 days. 4. Army Regulation 635-5 (Separation Documents) governs the preparation of the DD Form 214. It states the DD Form 214 is a summary of a Soldier's most recent period of continuous active service. It provides a brief, clear-cut record of active duty service at the time of release from active duty, retirement, or discharge. The regulation states: * Item 13 – will list all authorized awards * Item 12f – will list credit for all foreign service completed during the period covered by the DD Form 214 * Item 18 – for active duty Soldiers deployed with their units will list the entry, "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD -YYYYMMDD)" 5. Army Regulation 600-8-22 (Military Awards) states: * The Iraq Campaign Medal is authorized for service in direct support of Operation Iraqi Freedom from on or after 19 March 2003 through 31 December 2011; campaign participation included New Dawn from 1 September 2010 through 31 December 2011 * Bronze service stars are worn on campaign and service medals to denote participation in a named campaign DISCUSSION AND CONCLUSIONS: 1. DFAS records indicate the applicant was deployed to Kuwait. At the time, he would likely have been processed into the theater of operations through Kuwait which is likely why DFAS record show he served in that country. 2. The DFAS information is accepted as sufficient evidence with which to amend items 12f and 18 of his DD Form 214 to show he served in Kuwait/Iraq from 5 February through 13 December 2011, a period of 10 months and 8 days. 3. He met the criteria for award of the Iraq Campaign Medal with one bronze service star. These awards are not listed on his DD Form 214. 4. There is no evidence of record verifying he served in Kuwait/Iraq from January 2011 through February 2012. The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by: * deleting from item 12f of his DD Form 214 the entry, "0000-00-00" and adding the entry "0000-10-08" * adding to item 13 of this DD Form 214 the Iraq Campaign Medal with one bronze service star * adding to item 18 of this DD Form 214 the entry, "Service in Kuwait/Iraq: 20110205-20111213" 2. The Board further determined that the evidence presented is insufficient to warrant a portion of the requested relief.
