THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 31 December 2014 to show in items 19a (Mailing Address after Separation) and 19b (Nearest Relative) the address as "15522 Ashland Ave" instead of "15722 Ashland Ave." 2. The applicant states that items 19a and 19b are the same address. 3. The applicant provides her Illinois State Driver's License. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 1 July 1985 and was honorably discharged on 29 June 1995 by reason of entering Officer Training Program. On the next day, she was transferred to the U.S. Army Reserve Control Group (Reserve Officer Training Corps (ROTC)). 2. On 10 May 1997, the applicant was appointed as a Reserve commissioned officer in the rank of second lieutenant with a concurrent call to active duty. 3. The applicant provided a photocopy of her Illinois State Driver's License which shows her address as "15522 Ashland Ave." 4. A review of her military personnel documents in the interactive Personnel Electronic Records Management System (iPERMS) does not contain documents which reflect her address as "15522 Ashland Ave." 5. Army Regulation 635-8 (Separation Processing and Documents) prescribes policy and procedural guidance relating to transition management. It consolidates the policies, principles of support, and standards of service regarding processing personnel for transition. The instructions for preparing items 19a and 19b state that the data source is provided by the Soldier. The mailing address must be a permanent address. When a relative is not available list a close friend. Enter name, street address, city, state, and zip code. DISCUSSION AND CONCLUSIONS: 1. The applicant's request for correction of items 19a and 19b of her DD Form 214 for the period ending 31 December 2014 is acknowledged and determined to have merit. 2. The applicant's DD Form 214 for the period ending 31 December 2014 shows her mailing address after separation and her nearest relative as "15722 Ashland Ave" in items 19a and 19b. 3. The applicant's Illinois State Driver's License shows her address as "15522 Ashland Ave." 4. It appears that an administrative error may have occurred at the time her mailing address and address of her nearest relative were entered on her DD Form 214 for the period ending 31 December 2014. 5.
