THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 23 August 2013 to show a third award of the Joint Meritorious Unit Award. 2. The applicant states he was assigned to Headquarters (HQ), Special Operations Command Central (SOCOMC) from 5 September 2008 to 23 August 2013. On 12 August 2013, Joint Staff Permanent Order Number (JSPO) J-ISOR-00199-13 awarded HQ, SOCOMC the Joint Meritorious Unit Award for the period 1 November 2009 to 31 December 2012. This information may not have been available at the time his DD Form 214 was completed on 22 August 2013. He had previously been awarded the Joint Meritorious Unit Award for service during Operation Provide Comfort, as shown on his DD Form 214 for the period ending 31 August 1994 and for service in HQ, SOCOMC during the period 5 September 2008 to 31 October 2009. 3. The applicant provides two DD Forms 214; 18 orders, dated between 8 August 2008 and 12 August 2013; three citations; and two certificates. CONSIDERATION OF EVIDENCE: 1. The applicant’s records show he was appointed as a commissioned officer in the Regular Army on 2 June 1976 and he held area of concentration (AOC)/military occupational specialty (MOS) 18A (Special Forces Officer). He was promoted to the rank/grade of major (MAJ)/O-4 on 1 July 1987. He was honorably retired on 31 August 1994 in the rank of MAJ by reason of voluntary early retirement and he was transferred to the U.S. Army Reserve (USAR) Control Group (Retired Reserves). The DD Form 214 he was issued for this period of service shows in part, the Joint Meritorious Unit Award. 2. He was ordered to active duty from the USAR Retired Reserve in AOC/MOS 18A and he entered active duty on 24 August 2008. He was assigned to HQ, SOCOMC, MacDill Air Force Base (AFB), FL. Between 10 November 2008 and 3 July 2013, he completed numerous deployments to multiple countries throughout the Middle East with his assigned unit. 3. He was honorably released from active duty on 23 August 2013 in the rank of MAJ and he was transferred to the USAR Retired Reserves. The DD Form 214 he was issued for this period of service shows in part, the Joint Meritorious Unit Award (2nd Award). 4. The applicant provides letters from The Joint Staff dated: * 5 March 2010, wherein it stated JSPO Number J-1SO-0028-10 awarded the Joint Meritorious Unit Award to HQ, SOCOMC, for the period 1 June 2007 through 31 October 2012 * 12 August 2013, wherein it stated JSPO Number J-1SO-00199-13 awarded the Joint Meritorious Unit Award to HQ, SOCOMC, for the period 1 November 2009 through 31 December 2012 DISCUSSION AND CONCLUSIONS: The applicant’s DD Form 214 for the period ending 31 August 1994 confirms he was authorized the Joint Meritorious Unit Award (1st Award) and he provides orders that show HQ, SOCOMC was awarded two awards of Joint Meritorious Unit Award during the period of his assignment to the unit. He meets the criteria for correction of his DD Form 214 for the period ending 23 August 2013 to show the Joint Meritorious Unit Award (3rd Award).
