Application for correction of military records (with supporting documents provided, if any). 2. Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests the removal of two documents from his official military personnel file (OMPF), specifically a Certificate of Training for completing Expert Field Medical Badge (EFMB) standardization training from 4 May through 15 May 2013, and a Certificate of Achievement for earning the EFMB, dated 15 May 2013. 2. The applicant states his social security number (SSN) is incorrectly shown on these two documents. 3. The applicant provides a copy of the Certificate of Training and Certificate of Achievement. CONSIDERATION OF EVIDENCE: 1. The applicant was appointed as a Reserve commissioned officer of the Army, as a second lieutenant in the Medical Service Corps, on 12 August 2011. He executed DA Forms 71 (Oath of Office – Military Personnel) on 12 August 2011, as a Reserve commissioned officer and on 6 October 2011, as a Regular commissioned officer. 2. He was promoted to first lieutenant, effective 6 April 2013. 3. His OMPF contains both of the subject certificates; however, both certificates display his correct SSN. Versions of these certificates displaying an incorrect SSN were not found in his OMPF. 4. Army Regulation 600-8-104 (Army Military Human Resource Records Management) governs the composition of the OMPF and states the performance folder is used for filing performance, commendatory, and disciplinary data. Once placed in the OMPF, the document becomes a permanent part of that file. The document will not be removed from or moved to another part of the OMPF unless directed by certain agencies, to include this Board. DISCUSSION AND CONCLUSIONS: The applicant's OMPF contains both of the subject certificates; however, both certificates display his correct SSN. Versions of these certificates displaying an incorrect SSN were not found in his OMPF. Therefore, there is neither an error nor injustice and there is no basis for granting the requested relief.
