THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Joint Service Commendation Medal with Oak Leaf Cluster (JSCM with 1 OLC). 2. The applicant states this medal is his end of service award which was presented after his DD Form 214 was requested. 3. The applicant provides copies of: * Military Personnel Awards Order Number GA-15-065, dated 30 January 2015 * Citation for award of the JSCM with 1 OLC * Certificate for award of the JSCM with 1 OLC, dated 27 January 2015 CONSIDERATION OF EVIDENCE: 1. On 5 December 2002, the applicant entered the Regular Army. He attained the rank of major, pay grade O-4, effective 1 September 2012. 2. Military Personnel Awards Order Number GA-15-065, dated 30 January 2015, announced the applicant's award of the JSCM with 1 OLC, for the period 8 October 2012 through 30 April 2015. 3. On 1 May 2015, the applicant was discharged. His DD Form 214 shows that his personal decorations include one award of the JSCM. 4. Army Regulation 635-8 (Separation Processing and Documents) provides detailed instructions for completing separation documents, including the DD Form 214.  It provides that the DD Form 214 is a summary of a Soldier's most recent period of continuous active service. It provides a brief, clear-cut record of active duty service at the time of release from active duty, retirement, or discharge. The DD Form 214 is not intended to have any legal effect on termination of a Soldier's service. It provides that the entry in Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) will list all federally recognized awards and decorations for all periods of service. 5. Army Regulation 600-8-22 (Military Awards) states an oak leaf cluster is awarded to denote the second and succeeding awards of certain decorations, among which is the JSCM. DISCUSSION AND CONCLUSIONS: 1. The applicant contends that his DD Form 214 should be corrected to show the JSCM with 1 OLC. 2. The available evidence shows the applicant was awarded two JSCM's, but only the first award is listed on his DD Form 214. 3. In view of the above, it would be appropriate to correct his DD Form 214 by showing the JSCM with 1 OLC to indicate he is authorized a second award.
