THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, reconsideration of the previous Army Board for Correction of Military Records (ABCMR) decision set forth in Docket Number AR20130008482 on 28 January 2014. Specifically, he requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the Southwest Asia Service Medal and all applicable campaign medals and awards. 2. The applicant states he should be awarded the Southwest Asia Service Medal for his service in Saudi Arabia from December 1992 to 1993. 3. The applicant provides copies of his DD Form 214, a self-authored letter, a certificate of achievement, and a buddy statement with permanent orders belonging to his buddy. CONSIDERATION OF EVIDENCE: 1. Incorporated herein by reference are military records that were summarized by the ABCMR in the previous consideration of the applicant's case in Docket Number AR20130008482, on 28 January 2014. 2. The applicant enlisted in the Regular Army on 14 March 1991. Following the completion of his initial entry training, he was awarded military occupational specialty 11M (Fighting Vehicle Infantryman). 3. He was honorably released from active duty on 13 March 1994. The DD Form 214 he was issued at the time shows: * Block 12f (Foreign Service), the entry "0000-00-00." * Block 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized), he was awarded or authorized the: * Army Achievement Medal * Army Good Conduct Medal (1st Award) * National Defense Service Medal * Army Service Ribbon * Expert Infantryman Badge * Expert Marksmanship Qualification Badge with Rifle Bar * Expert Marksmanship Qualification Badge with Grenade Bar * Army Lapel Button * Block 18 (Remarks) is void of any entry related to his participation in any named campaigns during a period of credited foreign service. 4. The applicant provides: a. A Certificate of Achievement, wherein he is recognized for meritorious service during his assignment to the United States Army Forces Central Command, Saudi Arabia. b. A statement provided by a fellow Soldier attesting to his, as well as the applicant's, deployment to Saudi Arabia from on or about 10 December 1992 through on or about 4 April 1993. The witness provides copies of his deployment orders and release from attachment orders verifying his service in theater. 5. The Defense Manpower Data Center (DMDC) compiled the Operations Desert Shield/Desert Storm database, which is the primary file that contains one record for each active duty member who participated in theater. This database credits the applicant with service in Southwest Asia during the period 1 December 1992 through 30 April 1993. REFERENCES: 1. Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214. Chapter 2 contains guidance for preparation of the DD Form 214. It states that for: a. Block 12f, the preparer should enter the total active duty performed outside the continental United States during the period covered by the DD Form 214; b. Block 13, the preparer should list all awards and decorations from the Soldier's records; and c. Block 18, for a Soldier deployed with his or her unit during their continuous period of active service, the preparer should enter the statement "SERVICE IN (name of country deployed) FROM (inclusive dates for example, YYYYMMDD-YYYYMMDD)." 2. Army Regulation 600-8-22 (Military Awards) prescribes Army policy, criteria, and administrative instructions concerning individual and unit military awards. a. The Southwest Asia Service Medal is awarded to members of the Armed Forces of the United States who participated in Operations Desert Shield/Desert Storm in the designated area on or after 2 August 1990 to 30 November 1995. A bronze service star is authorized for wear with this medal for participation in each credited campaign. During his service in Saudi Arabia, he participated in the Cease-Fire Campaign (12 April 1991 to 30 November 1995). b. The Kuwait Liberation Medal, issued by the Government of Kuwait, was approved on 9 November 1995 and is awarded to members of the Armed Forces of the United States who participated in the Persian Gulf War between 2 August 1990 and 31 August 1993. DISCUSSION: 1. The applicant's request for correction of his DD Form 214 to show the Southwest Asia Service Medal and all applicable campaign medals and awards was carefully considered. 2. In support of his request, he provides a Certificate of Achievement for his service in Saudi Arabia and a witness statement. The DMDC database verifies his service in Southwest Asia during a qualifying period of service. a. Item 12f of his DD Form 214 does not reflect the 4 months and 29 days of foreign service he completed. b. He served a qualifying period of service for the Southwest Asia Service Medal with one bronze service star to designate his participation in one campaign; however, his DD Form 214 does not show this award or its corresponding bronze service star. c. He served a qualifying period of service for the Kuwait Liberation Medal awarded by the Government of Kuwait; however, his DD Form 214 does not show this foreign service award. d.
