THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests reconsideration of his request for correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 31 July 1999 to show his service in Kuwait from July 1992 to August 1993 and Turkey from May 1997 to July 1998 to item 18 (Remarks). 2. The applicant states he wants Kuwait and Turkey added to his DD Form 214 showing his complete service in Southwest Asia (SWA). 3. The applicant provides: * Orders Number 156-278, dated 4 June 1992 * Orders Number 97-48, dated 7 April 1993 * Orders Number 055-00417, dated 24 February 1997 * Orders Number 250-3, dated 3 September 1997 * Record of Proceedings (ROP), Army Board for Correction of Military Records (ABCMR) Docket Number AR20140021461, dated 28 July 2015 CONSIDERATION OF EVIDENCE: 1. Incorporated herein by reference are military records, which were summarized in the previous consideration of the applicant's case in ABCMR Docket Number AR20140021461 on 28 July 2015. 2. The applicant provides a series of military orders as new evidence the Board did not previously consider. This new evidence warrants consideration by the Board. 3. The applicant enlisted in the Regular Army on 30 June 1971 and was discharged on 16 March 1975 for immediate reenlistment; his DD Form 214 for this period is not present in his record. 4. He reenlisted on 17 March 1975. His DD Form 4 (Enlistment Contract - Armed Forces of the United States) for this reenlistment shows his characterization of service for the period 30 June 1971 to 16 March 1975 as honorable. 5. The Army honorably discharged him on 13 February 1979. The DD Form 214 issued for this period shows he served in Germany from 21 January 1972 to 21 June 1974 and from 30 July 1975 to 25 July 1978. He completed 2 years, 11 months, and 26 days of foreign service. 6. He reenlisted in the Regular Army on 14 February 1979. He was honorably discharged on 23 January 1981 and ordered to active duty as a warrant officer. His DD Form 214 for this period shows he completed 5 years, 4 months, and 27 days of foreign service. 7. On 14 January 1981, he was appointed as a Reserve warrant officer of the Army. 8. On 15 October 1985, he was appointed as a warrant officer in the Regular Army in the rank/grade of chief warrant officer two/W-2. He executed his oath of office on 19 November 1985. 9. His ORB contains the following entries: a. Section I (Assignment Information), shows: * assignment in Germany for 36 months returning July 1978 * assignment in Germany for 54 months returning December 1985 * assignment in Korea for 12 months returning December 1989 * assignment in Kuwait for 12 months returning August 1993 * assignment in Saudi Arabia for 6 months returning March 1996 * assignment in Turkey for 13 months returning July 1998 b. Section IX (Assignment History) shows: * assignment with the Security Assistance Training Management Organization, Kuwait, starting 31 July 1992 for a period of 14 months * assignment with the Ordnance Program Division, Saudi Arabia, starting 23 September 1995 for a period of 6 months * assignment with the U.S. Army Element, Allied Land Forces Southeastern Europe, Izmir, Turkey, starting 2 June 1997, for a period of 14 months 10. His service record contains three DA Forms 67-8 (U.S. Army Officer Evaluation Report) covering three rated periods from 29 October 1988 through 17 December 1989. These reports show he was assigned to Headquarters and Headquarters Company, 2nd Battalion, 72nd Armor, 2nd Infantry Division in Korea during the rated periods. 11. On 5 March 1990, he was appointed as a warrant officer in the Regular Army in the rank/grade of chief warrant officer three/W-3. 12. Orders Number 156-278, issued by Headquarters, U.S. Army Armor Center and Fort Knox, KY, on 4 June 1992 show he was to proceed on permanent change of station for assignment to the Assistance Training Teams at Fort Bragg, NC, with duty at the U.S. Embassy in Kuwait. The reporting date was in accordance with his port call; however, the port call date is not available. 13. Orders Number 97-48, issued by Headquarters, XVIII Airborne Corps and Fort Bragg, NC, on 7 April 1993 show he was to proceed on permanent change of station from the High Mobility Multipurpose Wheeled Vehicle (HMMWV) Technical Assistance Field Team in Kuwait to the Fort Knox Officer Replacement Detachment with a reporting date of not later than 30 September 1993. 14. Orders Number 055-00417, issued by Headquarters, U.S. Army Armor Center and Fort Knox, KY, on 24 February 1997 show he was to proceed on permanent change of station for assignment to U.S. Army Element, Allied Land Forces Southeastern Europe, located in Turkey, with a reporting date of 2 June 1997. 15. Orders Number 250-3, issued by Headquarters, U.S. Army Element, Allied Land Forces Southeastern Europe, on 3 September 1997 show he was to proceed on permanent change of station from the U.S. Army Element Allied Land Forces Southeastern Europe, Izmir, Turkey, for assignment to the U.S. Army Armor Center and Fort Knox with a reporting date of 30 July 1998. 16. He retired on 31 July 1999 for sufficient length of service. In an email dated 27 September 2016, the Defense Finance and Accounting Service (DFAS) confirmed his receipt of hostile fire pay (HFP)/imminent danger pay (IDP) and combat zone tax exclusion (CZTE): * from 31 July 1992 to 2 August 1993 (1 year and 2 days) for service in Kuwait * from 2 June 1997 to 2 July 1998 for service in Turkey (1 year, 1 month, and 1 day) REFERENCES: Army Regulation 635-5 (Separation Documents) prescribes policies and procedures for the completion of the DD Form 214. Regarding required entries in item 18, it essentially states, when an active duty Soldier has deployed during the period covered by the DD Form 214, the period(s) of deployment as well as the country(ies) to which he/she was deployed will be shown (emphasis added). DISCUSSION: 1. The applicant's ORB indicates he served in Kuwait (12 months) and Turkey (13 months). The orders provided by the applicant show he was assigned as part of a permanent change of station to both Kuwait and Turkey. 2. Army Regulation 635-5 requires entries in item 18 for deployments occurring within the period covered by the DD Form 214. 3. The applicant's service record is void of evidence that shows additional deployed service that should have been listed in item 18 of his DD Form 214.
