THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 1 June 2015 to show he was awarded the Meritorious Service Medal (MSM). 2. The applicant states the MSM was not on his last discharge (i.e., separation document). 3. The applicant provides a copy of his DD Form 214 and a copy of the DA Form 638 (Recommendation for Award) that awarded him the MSM, which is annotated with Permanent Orders Number 159-007, Headquarters, U.S. Army Special Forces Command (USASFC) (Airborne), Fort Bragg, NC, dated 18 June 2015. CONSIDERATION OF EVIDENCE: 1. The applicant was appointed as a Regular Army officer on 13 May 2006. He entered active duty on 3 June 2006 and was honorably released from active service on 1 June 2015. He attained the rank of captain (CPT). 2. Permanent Orders Number 159-007, issued by Headquarters, U.S. Army Special Forces Command (USASFC) (Airborne), Fort Bragg, NC on 18 June 2015, awarded the applicant the MSM for exceptionally meritorious service during the period 7 November 11 through 1 June 2015. This award is not listed on his DD Form 214. REFERENCES: Army Regulation 635-5 (Separation Documents), prescribes the DD Form 214 as a summary of the Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of all current active, prior duty, and prior inactive duty service at the time of released from active duty, retirement, or discharge. Block 13 requires the preparer to list all federally recognized awards and decorations for all periods of service.
