Application for correction of military records (with supporting documents provided, if any). 2. Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of her DD Form 214 (Certificate of Release of Discharge from Active Duty) to show: a. she held area of concentration (AOC) 92A (Quartermaster) for 4 years and 5 months instead of AOC 88A (Transportation) in item 11 (Primary Specialty) and b. award of the Joint Service Commendation Medal, three Army Commendation Medals, and three Army Achievement Medals. 2. The applicant states: a. Item 11 of her DD Form 214 currently shows AOCs 38A (Civil Affairs) and 88A, but it should show she held AOC 38A for 3 years and 6 months and AOC 92A for 4 years and 5 months. The error occurred during preparation of her DD Form 214. b. The Joint Service Commendation Medal should be added to item 13 (Decorations, Medals, Badges, Citations, and Campaign Ribbons Awarded or Authorized) of her DD Form 214. Her DD Form 214 shows two Army Commendation Medals, but she has three Army Commendations Medals. Her DD Form 214 also shows the Army Achievement Medal, but she has three Army Achievement Medals. The additional awards were presented after preparation of her DD Form 214. 3. The applicant provides: * 1st Brigade, Eastern Region, U.S. Army Cadet Command, memorandum, dated 18 May 2007 * Quartermaster Office Basic Course Diploma * Joint Service Commendation Medal Certificate * three Army Commendation Medal Certificates * three Army Achievement Medal Certificates CONSIDERATION OF EVIDENCE: 1. The applicant was appointed as a Reserve commissioned officer in the rank of second lieutenant on 18 May 2007 in the Quartermaster Corps. She entered active duty on 26 May 2007. 2. She provided: a. an undated Army Achievement Medal Certificate showing she was awarded the Army Achievement Medal for outstanding achievement as part of the Joint Readiness Training Center deployment exercise while serving in the rank of second lieutenant in 710th Brigade Support Battalion Permanent Order Number 156-001; b. an undated Army Achievement Medal Certificate showing she was awarded the Army Achievement Medal for achievement as the distinguished honor graduate for Air Assault Class 08-03 while serving in the rank of second lieutenant in 710th Brigade Support Battalion Permanent Order Number 337-019; c. an Army Achievement Medal Certificate, dated 13 August 2007, showing she was awarded the Army Achievement Medal for exceptionally meritorious achievement while serving as Assistant Operations Officer in the rank of second lieutenant during the period 2 June 2007 to 16 August 2007 in Western Region Reserve Officers' Training Corps Permanent Orders 221-04, dated 9 August 2007; and d. a U.S. Army Quartermaster School diploma, dated 8 February 2008, showing she successfully completed the Quartermaster Officer Basic Course. 3. She was promoted to captain effective 1 June 2010. 4. She provided an Army Commendation Medal Certificate, dated 14 June 2010, showing she was awarded the Army Commendation Medal for exceptional and meritorious service while assigned as the Supply Support Activity Platoon Leader, Alpha Company Executive Officer, and S-3 Plans and Operations Officer in the rank of captain during the period 1 February 2008 to 9 July 2010 in Headquarters, 3d Infantry Brigade Combat Team, 10th Mountain Division, Permanent Order Number 165-001. 5. Her DA Form 1059 (Service School Academic Evaluation Report), dated 3 July 2012, shows she completed Civil Affairs Qualification Course 05/06-11 during the period 22 August 2011-6 July 2012. 6. She also provided: a. an Army Commendation Medal Certificate, dated 17 November 2014, showing she was awarded the Army Commendation Medal for meritorious service as the Civil Affairs Team 8315 Team Leader while serving in the rank of captain during the period 5 September 2012 to 1 December 2014 in Headquarters, 85th Civil Affairs Brigade, Permanent Order Number 321-02, dated 17 November 2014; b. a Joint Service Commendation Medal Certificate, dated 17 August 2015, showing she was awarded the Joint Service Commendation Medal for meritorious service by Special Operations Command Central; and c. an Army Commendation Medal Certificate, dated 4 December 2015, showing she was awarded the Army Commendation Medal for exceptional service as a Civil Affairs Team Leader while serving in the rank of captain during the period 19 December 2014 to 1 January 2016 in Headquarters, 95th Civil Affairs Brigade (Airborne), Permanent Orders 307-01. 7. On 1 January 2016, she was released from active duty by reason of completion of required active service. 8. Her DD Form 214 shows in: * item 11 – * 38A2B Civil Affairs – 3 Years 6 Months * 88A Transportation, General – 4 Years 5 Months * item 13, in part – * Army Commendation Medal (2nd Award) * Army Achievement Medal 9. There is no evidence showing she was awarded or held AOC 88A. REFERENCES: Army Regulation 635-8 (Separation Processing and Documents) establishes standardized policy for preparation of the DD Form 214. The instructions for item 11 state to enter the titles of all AOCs awarded and held for at least 1 year during the current period of service and include the number of years and months held. For time determination, 16 days or more count as a month. Do not count the Basic Officer Leaders Course. For commissioned officers, specify the AOC consisting of two numbers and one letter. Up to two additional skill identifiers or special skill identifier are allowed immediately following the AOC. DISCUSSION: 1. Item 11 of the applicant's DD Form 214 shows she held AOC 88A for 4 years and 5 months. There is no evidence showing she was awarded or held AOC 88A at any time. 2. The evidence of record shows she held AOC 92A from the time she completed the Quartermaster Officer Basic Course in February 2008 until she completed the Civil Affairs Qualification Course in July 2012. Item 11 of her DD Form 214 does not show she held AOC 92 for 4 years and 5 months (period of service, less Officer Basic Leaders Course). 3. Her DD Form 214 shows she was awarded two Army Commendation Medals. The evidence of record shows she was awarded three Army Commendation Medals. 4. Her DD Form 214 shows she was awarded one Army Achievement Medal. The evidence of record shows she was awarded three Army Achievement Medals. 5. The evidence of record shows she was awarded the Joint Service Commendation Medal.
