THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests reconsideration of his earlier request to correct his military records to show his last name as A____. 2. The applicant states his last name should be A____ instead of W____. 3. The applicant provides: * birth certificate * social security card * Maryland driver's license * DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) CONSIDERATION OF EVIDENCE: 1. Incorporated herein by reference are military records which were summarized in the previous consideration of the applicant's case by the Army Board for Correction of Military Records (ABCMR) in Docket Number AR20060009995 on 20 March 2007. 2. The applicant's contention is new evidence that was not previously considered by the Board and warrants consideration at this time. 3. He was inducted into the Army of the United States on 24 June 1968. His DD Form 47 (Record of Induction), dated 24 June 1968, shows his last name as W____. 4. On 26 June 1968, he was honorably discharged for immediate enlistment in the Regular Army. His DD Form 214 for the period ending 26 June 1968 shows his last name as W____. Item 32 (Signature of Person Being Transferred or Discharged) of this DD Form 214 shows a legible signature and the last name W____. 5. He enlisted in the Regular Army on 27 June 1968 for a period of 3 years. His DD Form 4 (Enlistment Record – Armed Forces of the United States), dated 27 June 1968, shows his last name as W____. 6. On 4 May 1971, he was honorably released from active duty. His DD Form 214 for the period ending 4 May 1971 shows his last name as W____. On 20 March 2007, the ABCMR denied his request to correct his last name. REFERENCES: Army Regulation 635-5 (Separation Documents), in effect at the time of his release from active duty, prescribed the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It established standardized policy for preparation of the DD Form 214 and stated the DD Form 214 is a synopsis of the Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement, or discharge. DISCUSSION: 1. The applicant contends his military records should show his last name as A____ instead of W____. 2. Although his birth certificate and social security card show his last name as A____, the evidence of record shows he was inducted under the last name W____ and this last name was used exclusively during his military service. 3. The Army has an interest in maintaining the integrity of its records for historical purposes. The information in those records must reflect the conditions and circumstances that existed at the time the records were created.
