Reference the attached Army Board for Correction of Military Records Record of Proceedings, dated 3 October 2017, in which the Board members unanimously recommended denial of the applicant's request. 2. I have reviewed the findings, conclusions, and Board member recommendations. I find there is sufficient evidence to grant relief. Therefore, under the authority of Title 1O, United States Code, section 1552, I direct that all Department of the Army Records of the individual concerned be corrected by amending her DD Form 214 to reflect her last name as V_ as shown in her final decree of divorce. I direct no further correction be made to the record of the individual concerned. 3. Request necessary administrative action be taken to effect the correction of records as indicated no later than 16 February 2018. The applicant's DA Form 3286 (Statement for Enlistment – United States Army Enlistment Program), dated 16 September 2011, shows her last name as V____. 2. Her records contain a State of Arkansas County of Faulkner Marriage License, dated 21 September 2011, showing she married M____ C____ B____ on that date. 3. She enlisted in the Regular Army on 17 October 2011. 4. Her records contain a DA Form 4187 (Personnel Action), dated 19 October 2011, showing her request for name change to B____ was approved. 5. She provided a District Court, 169th Judicial District, Bell County, TX, Final Decree of Divorce, dated 30 April 2015, showing her last name changed to V____ in connection with her divorce. 6. Her Enlisted Record Brief, dated 30 June 2015, shows her last name as B____. 7. She retired by reason of permanent disability (enhanced) effective 29 June 2015. 8. Block 1 (Name) of her DD Form 214, shows her last name as B____. Block 18 (Remarks) contains the entry: "BLOCK 1: OTHER NAME(S) OF RECORD: V____, E____ M____ L____." 9. Her military records contain no evidence indicating she requested a name change or correction prior to her retirement. REFERENCES: Army Regulation 635-8 (Separation Processing and Documents) prescribes policy and procedural guidance relating to transition management. It establishes standardized policy for preparation of the DD Form 214. It states the DD Form 214 is a summary of the Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of all current active, prior active, and prior inactive duty service at the time of release from active duty, retirement, or discharge. The instructions for block 1 state to compare to the original enlistment contract or appointment order and review official record for possible name changes. If a name change has occurred, list other names of record in block 18. DISCUSSION: 1. The applicant's records show her last name as V____ when she applied for enlistment on 16 September 2011. She married and legally changed her last name to B____ prior to enlisting on 17 October 2011. 2. She provided a divorce decree changing her married last name of B____ to her maiden last name of V____ effective 30 April 2015. However, item 1 of her DD Form 214 for the period ending 29 June 2015 shows her last name as B____. 3. Although item 18 of her DD Form 214 shows she served under the last name V____ during the period covered, the DD Form 214 should represent the circumstances as they exist at the time of release from active duty, retirement, or discharge. 4.
