Evidence and Consideration BOARD DATE: 14 June 2016 DOCKET NUMBER: AR20160009018 BOARD DETERMINATION/RECOMMENDATION: The Board determined the evidence presented is sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by removing the diploma for completion of the Aviation Operations Specialist Senior Leaders Course, dated 6 March 2015, from his Official Military Personnel File. ___________x______________ CHAIRPERSON I certify that herein is recorded the true and complete record of the proceedings of the Army Board for Correction of Military Records in this case. BOARD DATE: 14 June 2016 DOCKET NUMBER: AR20160009018 THE BOARD CONSIDERED THE FOLLOWING EVIDENCE: 1. Application for correction of military records (with supporting documents provided, if any). 2. Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests removal of a diploma for completion of the Aviation Operations Specialist Senior Leaders Course (SLC), dated 6 March 2015, from his Official Military Personnel File (OMPF). 2. The applicant states: a. He has two diplomas for completion of the SLC in his records. The correct diploma is dated 1 May 2015. b. He graduated from the SLC for military occupational specialty (MOS) 15P (Aviation Operations Specialist) on 1 May 2015. He did not notice the incorrect date on the diploma and he gave it to his S-1 (personnel officer). He received the correct diploma about a month later. 3. The applicant provides two diplomas, dated 6 March 2015 and 1 May 2015. CONSIDERATION OF EVIDENCE: 1. The applicant is currently serving on active duty in the Regular Army in the rank of staff sergeant. 2. He provided a diploma showing he completed the MOS 15P SLC on 6 March 2015. 3. His records contain a DA Form 1059 (Service School Academic Evaluation Report), dated 1 May 2015, showing he attended Aviation Operations Specialist SLC Class 15-003 from 31 March 2015 to 1 May 2015 and achieved course standards. 4. He provided a diploma showing he completed the MOS 15P SLC on 1 May 2015. 5. A review of the performance folder in his OMPF in the interactive Personnel Electronic Records Management System (iPERMS) revealed a copy of the two diplomas, dated 6 March 2015 and 1 May 2015. REFERENCES: Army Regulation 600-8-104 (Army Military Human Resource Record Management) prescribes Army policy for the creation, utilization, administration, maintenance, and disposition of the OMPF. Appendix B (Documents Authorized for Filing in the Army Military Human Resource Record and/or iPERMS) and the U.S. Army Human Resources Command website provide a listing of documents authorized for filing in iPERMS. These sources state to file course completion certificates for 40 or more hours in the performance folder of the OMPF. Paragraph 3-6 provides that once a document is properly filed in the OMPF, the document will not be removed from the record unless directed by the Army Board for Correction of Military Records or other authorized agency. DISCUSSION: 1. The applicant contends he graduated from the Aviation Operations Specialist SLC on 1 May 2015 and his DA Form 1059 supports his contention. 2. The evidence of record shows the MOS 15P SLC diploma, dated 6 March 2015, was issued erroneously and should not be filed in his OMPF. //NOTHING FOLLOWS// ABCMR Record of Proceedings AR20150000953 Enclosure 1 ARMY BOARD FOR CORRECTION OF MILITARY RECORDS RECORD OF PROCEEDINGS ABCMR Record of Proceedings (cont) AR20160009018 2 ARMY BOARD FOR CORRECTION OF MILITARY RECORDS RECORD OF PROCEEDINGS Enclosure 1 ABCMR Record of Proceedings (cont) AR20160009018 2 ARMY BOARD FOR CORRECTION OF MILITARY RECORDS RECORD OF PROCEEDINGS Enclosure 2
