THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty, ending on 27 May 2014, to show the: * Primary Leadership Development Course * Cannon Fire Direction Chief Course * TATS (Total Army Training System) Cannon Crewmember Course * Field Artillery (FA) Platoon Sergeant Course * Advanced Noncommissioned Officer (NCO) Course, Phase I and II * First Sergeant Course 2. The applicant states his DD Form 214 was processed without his review. He was hospitalized and retired due to disability. 3. The applicant provides 7 DD Forms 1059 (Service School Academic Evaluation Report). CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army (RA) on 11 August 1988 and held military occupational specialty (MOS) 13E (Cannon Fire Direction Specialist). He served through multiple reenlistments. 2. He was honorably discharged on 17 March 2001. His DD Form 214 for this period of service shows in item 14 (Military Education): * 13E Cannon Specialist Course, 7 weeks, 1989 * German Headstart, 1 week, 1989 * NCO Education System Primary, 4 weeks, 1994 * NCO Education System Basic, 6 weeks, 1998 * Unit Alcohol and Drug Coordinator Training Course, 1 week, 1999 3. He enlisted in the Pennsylvania Army National Guard (PAARNG) on 15 August 2003. He entered active duty on 27 June 2006. He was honorably released from active duty on 14 March 2007. His DD Form 214 for this period of service shows in item 14: * PALADIN CO Course, 2 weeks, 2006 * Advanced NCO Course - FA Platoon Sergeant - Phase I, 2 weeks, 2006 * Advanced NCO Course - FA Platoon Sergeant - Phase II, 2 weeks, 2006 * Unit Readiness NCO Course, 2 weeks, 2006 * Master Trainer Course, 6 weeks, 2005 4. He reentered active duty on 15 March 2007. He served in a variety of assignments and he attained the rank/grade of master sergeant (MSG)/E-8. He also completed the: * 2-week Total Army Instructor Training Course, from 28 May 2007 to 8 June 2007 * 2-week First Sergeant Course, 3 to 17 December 2011 5. He was retired by reason of disability on 27 May 2014 and he was placed on the retired list in his retired rank/grade of MSG/E-8 on 28 May 2014. Item 14 of his DD Form 214 for this period of serviced does not list any training courses. REFERENCES: Army Regulation 635-8 (Separation Processing and Documents) prescribes policy and procedural guidance relating to transition management. Chapter 5 provides for the preparation of the separation document. The DD Form 214 is a summary of the Soldier’s most recent period of continuous active duty. It provides a brief, clear-cut record of all current active, prior active, and prior inactive duty service at the time of release from active duty, retirement, or discharge. Block 14 (Military Education) requires a listing of all formal, in-service (full time attendance) training courses successfully completed during the period of service covered by the DD Form 214 of at least 1 week or 40 hours duration. DISCUSSION: 1. The applicant completed three periods of active duty and he was issued a separate DD Form 214 for each period of service 2. He first served on active duty from 11 August 1988 to 17 March 2001 and he was issued a DD Form 214 for this period. He completed several training courses during the period covered by this DD Form 214, including the Primary Leadership Development Course and Cannon Fire Direction Chief Course, and other training courses. These courses are listed in item 14 of his DD Form 214 covering the period. 3. He then served on active duty as a member of the PAARNG from 27 June 2005 to 14 March 2007 and he was issued a DD Form 214 for this period. He completed several training courses during the period covered by this DD Form 214. These courses are listed in item 14 of his DD Form 214. 4. He then served on active duty as a member of the PAARNG from 15 March 2007 to 27 May 2014 and he was issued a DD Form 214 for this period. He completed two training courses during the period covered by this DD Form 214: the 2-week Total Army Instructor Training Course, from 28 May to 8 June 2007 and the 2-week First Sergeant Course from 3 to 17 December 2011. Neither training course is listed on his DD Form 214. 5.
