Application for correction of military records (with supporting documents provided, if any). 2. Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, removal of four documents from his Army Military Human Resource Record (AMHRR) and Official Military Personnel File (OMPF). 2. The applicant states there are four unreadable documents filed in the performance folder of his AMHRR/OMPF. Three contain the file name "AWD DOC" (two dated 1 November 2011 and one dated 28 February 2008) and one contains the file name "DA [Form] 87 "(Certificate of Training), dated 1 November 2011. a. He states no other documents share these dates. He attempted to have this corrected through his S-1; however, the matter could not be remedied. He adds that he submitted readable copies of the documents and they are now filed in his AMHRR/OMPF. b. He believes the four unreadable documents were carelessly scanned and uploaded into his AMHRR/OMPF. A review of the documents will show they cannot be read and, therefore, should be removed from his AMHRR/OMPF. c. Note: The documents under review pertain to a DA Form 4980-14 (Army Commendation Medal (ARCOM) Certificate), DA Form 4980-18 (Army Achievement Medal (AAM) Certificate), Army Good Conduct Medal (AGCM) permanent order (PO), and DA Form 87 (Certificate of Training). 3. The applicant provides a copy of one of the unreadable documents. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 19 November 2003. He is currently serving on active duty in the rank of sergeant first class/pay grade E-7. 2. Table B-1 (Authorized Documents) provides guidance for filing records and documents in the OMPF. * Approved award documents will be filed in performance section of the OMPF. * File only certificates issued by activities listed in Army Training Requirements and Resources System Course Catalog. File as a "CRS CMPL 40" or a "CRS CMPL LESS 40 HRS" based on course length. b. Chapter 3 (AMHRR Management Program), paragraph 3-6 (Authority for filing or removing documents in the AMHRR folders), shows that only documents pertaining to a Soldier's military career will be filed in the OMPF. Once properly filed in the OMPF, the document will not be removed from the record unless directed by, in pertinent part, one of the following – * the Official Records Custodian for administrative purposes (this includes, but is not limited to deleting or moving mistakenly filed documents from the performance or service folder to the restricted folder and vice versa) * Army Board for Correction of Military Records DISCUSSION: 1. The applicant contends, in effect, that four unreadable documents should be removed from his AMHRR/OMPF because readable copies of the documents are filed in his AMHRR/OMPF and, therefore, the documents serve no purpose. 2. The evidence of record shows that four unreadable documents are filed in the applicant's AMHRR/OMPF and that readable copies of all four of the documents are also filed in his AMHRR/OMPF. 3.
