As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding to his DD Form 214 for the period ending 9 May 2001: * Master Fitness Trainer Course * Primary Leadership Development Course * Quarry Specialist Course 2. The Board further determined the evidence presented is insufficient to warrant a portion of the requested relief. Application for correction of military records (with supporting documents provided, if any). 2. Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to add the following entries to Item 14 (Military Education): * Basic Combat Training (BCT) * Quarry Specialist (Advanced Individual Training (AIT)) * Cavalry Scout (AIT) * Primary Leadership Development Course (PLDC) * Sapper Leader Course * Master Fitness Trainer (MFT) Course 2. The applicant states, in effect, that not all of his military education was added to his DD Form 214 while he was attending the Officer Candidate School due to a lack of supporting documentation at that time. 3. The applicant provides: * DD Form 214 for the period ending on 9 May 2001 * BCT certificate of training * Quarry Specialist Course certificate of training * Armored Reconnaissance Specialist certificate of training (Cavalry Scout) * DA Form 1059 (Service School Academic Evaluation Report) for PLDC * Sapper Leader Course certificate of graduation * Master Fitness Trainer Course certificate of training CONSIDERATION OF EVIDENCE: 1. After prior enlisted service, the applicant was appointed as a second lieutenant in the Regular Army on 10 May 2001. 2. A DD Form 214 shows he was honorably released from active duty on 9 May 2001 to accept a commission in the Regular Army. The DD Form 214 issued at that time shows he completed 5 years, 3 months, and 10 days of net active service. It also shows in Item 14 (Military Education), the entry "Officer Candidate School, 14 weeks, May 2001." 3. On 28 February 2017, he retired. His DD Form 214 shows he completed 15 years, 9 months, and 21 days of net active service this period. Item 14 does not list any of the requested military education. 4. He provides: a. A certificate, dated 4 April 1996, showing he completed BCT. b. A certificate, undated, showing he completed the 62G Quarrying Specialist Course. c. A certificate, dated 5 June 1997, showing he completed the Armored Reconnaissance Specialist Course. d. A DA Form 1059, dated 28 May 1998, showing he completed PLDC. e. A certificate, dated 3 November 1998, showing he completed the Sapper Leader Course. f. A certificate, dated 21 May 1999, showing he completed the Master Fitness Trainer Course. REFERENCES: Army Regulation 635-5 (Separation Documents) at the time established the standardized policy for preparing and distributing the DD Form 214. The DD Form 214 is a summary of a Soldier's most recent period of continuous active duty and will be prepared for all personnel at the time of retirement, discharge, or release from active duty. Item 14 shows formal in-service training courses completed during the period covered by the DD Form 214; courses for combat skills are not required to be listed. DISCUSSION: 1. Army Regulation 635-5 requires the entry of formal in-service training courses in item 14 of the DD Form 214; with the exception of training courses for combat skills, which it states are not entered. 2. BCT and the Cavalry Scout and Sapper Leader course are courses in combat skills. 3. MFT, PLDC, and the Quarry Specialist course are not combat courses.
