It establishes standardized policy for the preparation of the DD Form 214. A DD Form 214 will be prepared for all personnel at the time of their retirement, discharge, or release from active duty. Included are ARNG personnel who are separated after completing 90 days or more of continuous active duty for training, full time duty training, or active duty support; and after completing initial active duty for training that resulted in the award of a Military Occupational Specialty, even though the active duty was less than 90 days. a. The DD Form 214 is a summary of a Soldier's most recent period of continuous Active Duty. b. Detailed instructions for completing each item of the DD Form 214 stated: * item 4a (Grade, Rate or Rank) - enter active duty grade and pay grade at time of separation * item 6 (Reserve Obligation Termination Date) - this is the completion date of the statutory military service obligation incurred by a Soldier on initial enlistment in the Armed Forces, Soldiers with no previous military service who enlisted on or after 1 June 1984 to serve a period of 8 years * item 8a (Last Duty Assignment and Major Command) - enter last unit of assignment * item 8b (Station Where Separated) - enter location where Soldier is transitioning, include State or country * item 9 (Command to Which Transferred) - enter the applicable location dependent on the Soldier's status on transition * item 11 (Primary Specialty) - enter the titles of all MOSs served for at least 1 year, do not count basic training and advance individual training * item 12a (Date Entered Active Duty This Period) - enter the beginning date of the continuous period of Active Duty for issuance of this DD Form 214 * item 12b (Separation Date This Period) - the Soldier's transition date * item 12e (Total Prior Inactive Service) - enter the total amount of prior inactive service * item 12i (Effective Date of Pay Grade) - enter the effective date of promotion to the current pay grade * item 13 (Decorations Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) - list awards and decorations for all periods of service in the priority sequence * item 14 (Military Education) - list formal in-service (full-time attendance) training courses successfully completed during the period of service covered by the DD Form 214 * item 19a (Mailing Address After Separation) - this address must be a permanent address provided by the Soldier FACTS: 1. The applicant states: a. There are still many mistakes that are still not corrected on his DD Form 214. b. He is still struggling to get his discharge date to accurately reflect his service. He served in the military for 12 straight years. His official discharge date was 24 November 2001. Exactly 12 years after he enlisted. c. He is attaching a copy of his ribbons to show his awards he received during his 12 years of service. It is all he has to show proof of his awards that are not shown on his DD Form 214. d. The documents he has attached show his end date of his service as 24 November 2001 and his rank/grade as specialist/E-4. e. His service record show he enlisted in the Army National Guard on 25 November 1989. 4. His DA Form 87 (Certificate of Training) shows he completed Basic Training on 24 May 1990. 5. The Quartermaster School diploma, dated 5 July 1990, shows he completed the Laundry and Bath Specialist Course. 6. He was released from active duty training on 5 July 1990. There are no provisions allowing for the addition of achievements accomplished subsequent to the period covered by a DD Form 214. 8. There was no evidence in the applicant's service record of additional active duty periods with the issuance of a DD Form 214. 9. He attached documents to his reconsideration request showing his accomplishments and service in the Army National Guard after he was released from his initial active duty training. 10. His service record shows he was promoted to the rank/grade of specialist/E-4 on 1 October 1990. 11. His DA Form 4836 (Oath of Extension of Enlistment or Reenlistment), dated 24 November 1998, shows his extension for a period of 3 years and his new Expiration of Term of Service of 24 November 2001. 12. He separated from the Army National Guard on 24 November 2001. His National Guard Form 22 shows in: * item 4 (Date of Enlistment) - 25 November 1989 * item 5 (Rank & Pay Grade) - Specialist/E-4 * item 6 (Date of Rank) - 1 October 1990 * item 10a (Net Service This Period) - 12 years * item 11 (Terminal Date of Reserve/Military Service Obligation) - 24 November 2001 * item 12 (Military Education) - Infantryman, 5 Weeks, August 1998 * item 13 (Primary Specialty Number, Title and Date Awarded) - 13B10 Infantryman, 20 August 1997 * item 15 (Decorations Medals, Badges, Commendations, Citations and Campaign Ribbons Awarded) - Army Service Ribbon 13. On 9 December 2019, the ABCMR returned his application without action being taken by the Board. The applicant did not provide evidence showing he completed a period of active duty which required the issuance of a new DD Form 214. BOARD DISCUSSION: After reviewing the application and all supporting documents, the Board found that relief was not warranted. The Board carefully considered the applicant's request, supporting documents, evidence in the records and regulatory guidance. The Board considered the applicant's statement, his military record of service and documents provided by the applicant. The evidence in the record, shows there is no additional active duty periods with the issuance of a DD Form 214. The Board determined the documents the applicant provided show his accomplishments and service in the Army National Guard after he was released from his initial active duty training. Based upon regulatory guidance a DD Form 214 will be prepared for all personnel at the time of their retirement, discharge, or release from active duty. Included are ARNG personnel who are separated after completing 90 days or more of continuous active duty for training, full time duty training, or active duty support; and after completing initial active duty for training that resulted in the award of a Military Occupational Specialty, even though the active duty was less than 90 days. The Board did not find any evidence of error, injustice, or inequity; the DD Form 214 provided at his time of separation, correctly reflects his achievements received while on active duty.
