FACTS: 1. The Board will not consider award of the Army Good Conduct Medal (AGCM) as this item will be addressed through an administrative correction. 2. The applicant states, in effect, her divorce was finalized on 19 May 2014. She further claims to have provided a copy of her social security card and divorce decree; however, none were present in the record. 3. The applicant enlisted in the Regular Army (RA) on 30 April 1982. 4. Her records contain a License and Certificate of Marriage, which shows she married "B" on 2 October 1986. 5. She was honorably released from active duty on 26 November 1986. She served 4 years, 6 months and 27 days net active service during this period. The DD Form 214 she was issued lists her last name as "B." It further shows she was awarded or authorized the: * Army Service Ribbon * Overseas Service Ribbon * Army Achievement Medal * Sharpshooter Marksmanship Qualification Badge with Rifle Bar (M16) 6. The applicant reenlisted in the RA on 8 January 1997. Evidence shows she served in Iraq from 30 April to 4 October 2003 and 25 January to 15 May 2004, in Bosnia from 1 September 1999 to 2 March 2000, and Kosovo from 4 February to 26 May 2001. On 4 January 2005, she was honorably discharged at the completion of her required active service. The DD Form 214 she was issued lists her last name as "B." The applicant enlisted in the Army National Guard on 7 February 2013. On 13 March 2014, she was released to the U.S. Army Reserve (USAR) Control Group (Annual Training) due to unsatisfactory participation. On 31 July 2018, she was honorably discharged from the USAR. 8. The DD Form 214 is a synopsis of the Soldier's most recent period of continuous active service. It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement, or discharge. Item 1 NAME (Last, First, Middle) states to transcribe the name at the time of separation from applicable documents. BOARD DISCUSSION: 1. The Board concurred with the corrections addressed in Administrative Note(s) below. 2. The Board noted the applicant consistently used the last name shown her DD Form 214 subsequent to her marriage in 1986. The Board agreed that a post-service divorce is not a sufficiently mitigating factor to support changing the last name on her DD Form 214, which correctly reflects the information that was available when the form was completed. The Board determined the last name recorded on her DD Form 214 is not in error or unjust. 3. The applicant is advised that a copy of these proceedings will be filed in her Official Military Personnel File, which should serve to explain the difference in the name on her DD Form 214 and the name she now uses.
