APPLICANT'S SUPPORTING DOCUMENTS CONSIDERED BY THE BOARD: .DD Form 149 (Application for Correction of Military Record) .DD Form 214 .State Marriage License, issued 14 March 2019 FACTS: 1.The applicant states that she was recently married and changed her last name; shewants her military records to reflect her current legal name. 2.Her military records show she enlisted in the Regular Army on 5 November 2013.Her enlistment contract reflects her contested name.. 3.On 3 April 2018, she was medically retired. Her DD Form 214 lists her contestedname. The name listed on her DD Form 214 is the only name recorded in her militaryrecords. 4.The applicant provides a marriage license, which shows she married on 17 March2019. 5.Army Regulation 635-8 (Separation Processing and Documents) states, in effect,that the DD Form 214 will reflect the conditions and circumstances that existed at thetime the records were created. 6.For historical purposes, the Army has an interest in maintaining the integrity of itsrecords. The data and information contained in those records should reflect theconditions and circumstances that existed at the time the records were created. BOARD DISCUSSION: After review of the application and all evidence, the Board determined there is insufficient evidence to grant relief. The applicant’s name change occurred after the period of service covered by the DD Form 214. Without any extenuating factors other than a name change due to marriage, the Board agreed regulatory guidance provides for the maintaining of military records as they were during the member’s period of service. The DD Form 214 is of vital importance to the separating Soldier and must be properly prepared according to prescribed guidance.
