IN THE CASE OF: BOARD DATE: 20 August 2021 DOCKET NUMBER: AR20210006493 APPLICANT REQUESTS: correction of her Integrated Personnel Electronic Records Management System (iPERMS) record to show removal of DA Form 1506 (Statement of Service for Computation of Length of Service for Pay Purposes), dated 18 June 2014. The applicant states the Joint Base Lewis McChord, I Corps, career counselor/retention noncommissioned officer in 2017 realized that there was not a DA Form 1506 in the Defense Finance and Accounting Service (DFAS) system even though the 2014 one was uploaded into her iPERMS. He took a closer look and realized it was not calculated correctly. She then went through the correct steps to calculate and create a new DA Form 1506 that accurately reflects her U.S. Army Reserve (USAR) service, and he had it certified/verified by DFAS. DFAS approved it, and that correct DA Form 1506 is now in her iPERMS with the effective date of 6 September 2018. Once this took place, she began working on trying to get the incorrect one removed from her record. The process she went through is detailed in the enclosed email thread, dated 22 October 2020 to a at HRC. Her reply then at the top of the thread is her official denial notice stating that HRC is unable to remove documents from iPERMS, so it must go through the Board instead. 3. On 21 May 2020, the applicant was appointed as a Regular Army commissioned officer and executed an oath of office. 4. The applicant records contain the following: * DA Form 1506, dated 18 June 2014, showing her USAR service from 2011 – 2014 * DA Form 1506, dated 6 September 2018, showing her USAR service from 2010 - 2014 5. The applicant remains on active duty. 6. The applicant provides: a. Email correspondence with HRC wherein she informed by an HRC representative that the document was properly filed and they are not authorized to remove it. She must petition the Board. b. Applicant letter to HRC wherein she requests to have the incorrect DA Form 1506 removed from her record. c. ABCMR letter wherein the applicant was directed to HRC to attempt to have the incorrect DA Form 1506 removed from her record. BOARD DISCUSSION: After reviewing the application, all supporting documents, and the evidence found within the military record, the Board found that relief was warranted. The applicant’s contentions, the military record, and regulatory guidance were carefully considered. The Board agreed the contested DA Form 1506 does not reflect accurate information as a new form was completed to make her record whole. The Board determined there is sufficient evidence to grant relief.
