Having 4 years of prior active service, the applicant enlisted into the Regular Army on 30 April 2008. Item 1 (NAME (Last, First, Middle)) of his DD Form 4 (Enlistment/Reenlistment Document - Armed Forces of the United States) lists the contested name. 2. The applicant was honorably discharged on 27 March 2012. His record contains a DD Form 214 (Certificate of Release or Discharge from Active Duty), which lists the contested name. 3. The applicant provides a copy of a Judgment Entry - Change of Name of Adult, which legally changed his name as he requests effective 16 July 2020. 4. The applicant's military record did not contain any documentation that shows the name he now requests. 5. The DD Form 214 is a synopsis of the Soldier's most recent period of continuous active service. It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement, or discharge. BOARD DISCUSSION: After reviewing the application and all supporting documents, the Board determined relief was not warranted. Based upon the available documentation showing the applicant changed his name post-service and the DD Form 214 is responsible for recording information at the time of its publication, the Board concluded there was insufficient evidence of an error or injustice which would warrant a change to the applicant’s name reflected on his DD Form 214.
