Facts, Circumstances, and Legal Basis for Separation a. Facts and Circumstances: The evidence of record shows that on 30 July 2007, the applicant voluntarily requested separation from the Army under the provisions of AR 635-200, Chapter 4, Paragraph 4, by reason of completion of required active service. However, the complete facts and circumstances pertaining to the applicant’s discharge from the Army are not contained in the available records. The record does contain a properly constituted DD Form 214 (Certificate of Release or Discharge from Active Duty), which was authenticated by the applicant’s signature. His DD Form 214 indicates that he was released from the Regular Army under the provisions of AR 635-200, Chapter 4, by reason of completion of required active service, with a honorable discharge.
