Pursuant to the provisions of reference (a) Subject, hereinafter referred to as Petitioner, filed enclosure (1) with this Board requesting, in effect, that the applicable naval record be corrected to reflect Petitioner’s Home of Record (HOR) as , at the time of initial enlistment. 2. The Board, consisting of , reviewed Petitioner’s allegations of error and injustice on 17 January 2020 and, pursuant to its regulations, determined that the corrective action indicated below should be taken on the available evidence of record. Documentary material considered by the Board consisted of the enclosures, relevant portions of Petitioner’s naval record, and applicable statutes, regulations and policies. 3. The Board, having reviewed all the facts of record pertaining to Petitioner’s allegations of error and injustice, finds as follows: a. Before applying to this Board, Petitioner exhausted all administrative remedies available under existing law and regulations within the Department of the Navy. b. In correspondence attached as enclosure (2), the office having cognizance over the subject matter addressed in Petitioner’s application has commented to the effect that the request has merit and warrants favorable action. c. Petitioner graduated from High School in , in May 1978. d. Petitioner graduated from College in , in August 1983. e. Petitioner enlisted in the Naval Reserve in , on 3 February 1984. Note: DD Form 4, Enlistment/Reenlistment Document reflects HOR as , and , is reflected on DD Form 1966, Application for Enlistment as the HOR. f. Subsequently, Petitioner’s DD Form 1966, Application for Enlistment, block 28 is amended to reflect a address from 06/84 through 01/85. g. Petitioner’s active duty service date was 3 January 1985. h. Petitioner was discharged on 18 June 1992. CONCLUSION Upon review and consideration of all the evidence of record, and especially in light of the contents of enclosure (2), the Board finds the existence of an injustice warranting the following corrective action. Although the proper administrative requirements were not completed, the Board concluded Petitioner’s record contains sufficient evidence reflecting HOR was Pottsboro, Texas upon accessing in the Navy. RECOMMENDATION That Petitioner’s naval record be corrected, where appropriate, to show that: DD Form 4, Enlistment/Reenlistment Document, executed on 3 February 1984 is modified to reflect block 5 (Home of Record) “ ” vice “ ”. DD Form 1966, Application for Enlistment, executed on 3 February 1984 is modified to reflect block 4 (Home of Record) “ ” vice “ ”. A copy of this Report of Proceedings will be filed in Petitioner’s naval record. 4. It is certified that quorum was present at the Board’s review and deliberations, and that the foregoing is a true and complete record of the Board’s proceedings in the above entitled matter. 5.
