Pursuant to the provisions of reference (a), Petitioner, a former enlisted member of the U.S. Navy, filed enclosure (1) requesting an administrative change to his Armed Forces of the United States Report of Transfer of Discharge form (DD Form 214N) to reflect his correct Social Security Number (SSN). Enclosures (1) and (2) apply. 2. The Board, consisting of , reviewed Petitioner's allegations of error and injustice on 2 October 2020, and, pursuant to its regulations, determined that the corrective action indicated below should be taken on the available evidence of record. Documentary material considered by the Board consisted of the enclosures, relevant portions of his naval service records, and applicable statutes, regulations, policies. 3. The Board, having reviewed all the facts of record pertaining to Petitioner's allegations of error and injustice finds as follows: a. Before applying to this Board, Petitioner exhausted all administrative remedies available under existing law and regulations within the Department of the Navy. b. Although enclosure (1) was not filed in a timely manner, it is in the interests of justice to review the application on its merits. c. The Petitioner enlisted in the U.S. Navy and began a period of active service on 9 February 1970. On 6 April 1971, Petitioner was administratively separated from active duty by reason of an inability to adapt to military service with a general (under honorable conditions) characterization of service. The Petitioner’s USN service number was D107610. d. The SSN as reflected on Petitioner’s DD Form 214N is “.” However, the Social Security Administration (SSA) informed him on 2 March 2020 that his SSN was inadvertently assigned to two individuals and that his correct SSN is “.” The e. Petitioner provided a copy of his new Social Security card reflecting his correct SSN, as well as the letter from the SSA informing Petitioner of the error. CONCLUSION: Upon review and consideration of all the evidence of record, the Board concludes that Petitioner’s request warrants relief. The Board noted that the last digit of the Petitioner’s SSN as reflected on his DD Form 214N is incorrect. RECOMMENDATION: In view of the foregoing, the Board finds the existence of an error warranting the following corrective action. That Petitioner be issued a “Correction to DD Form 214, Certificate of Release or Discharge from Active Duty” (DD Form 215), to indicate that his SSN in Block 3 is changed to “.” Following the correction to the DD Form 214 indicating the correct SSN, that all other information currently listed on such DD Form 214 remain the same. That in the event that Petitioner’s DD Form 214 is obsolete and cannot be corrected through the use of a DD Form 215, that Petitioner be issued a new DD Form 214, Certificate of Release or Discharge from Active Duty, reflecting the recommended correction above. That a copy of this report of proceedings be filed in Petitioner’s naval record. That, upon request, the Department of Veterans Affairs be informed that Petitioner's application was received by the Board on 30 December 2019. 4. It is certified that a quorum was present at the Board’s review and deliberations, and that the foregoing is a true and complete record of the Board’s proceedings in the above entitled matter. 5.
