Pursuant to the provisions of reference (a), Petitioner, a former Sailor, filed enclosure (1) with the Board for Correction of Naval Records (Board) requesting a change to his Certificate of Release or Discharge from Active Duty (DD Form 214) to reflect his entitlement to the Submarine Warfare Pin, and the Navy Unit Commendation Medal. 2. The Board, consisting of , reviewed Petitioner's allegations of error and injustice on 14 April 2021, and, pursuant to its regulations, determined the corrective action indicated below should be taken on the available evidence of record. Documentary material considered by the Board consisted of the enclosures, relevant portions of his naval service records, and applicable statutes, regulations, policies. 3. The Board, having reviewed all the facts of record pertaining to Petitioner's allegations of error and injustice, finds as follows: a. Before applying to this Board, Petitioner exhausted all administrative remedies available under existing law and regulations within the Department of the Navy. b. Although enclosure (1) was not filed in a timely manner, it is in the interest of justice to review the application on its merits. c. Petitioner enlisted in the Navy Reserves on 16 March 1963. Petitioner commenced a period of active duty on 26 August 1964 and was assigned to the . While serving on the , the Petitioner’s unit was awarded the Navy Unit Commendation medal for the period from September 1964 to September 1966. Additionally, during his service, on 2 October 1965, Petitioner qualified as a submariner. d. Petitioner was discharged from active duty on 21 September 1966, and returned to the reserve component. Block 26 of his Armed Forces of the United States Report of Transfer or Discharge (DD Form 214) for the period ending on 15 March 1970 lists the National Defense Service Medal. e. On 26 June 2013, Petitioner was issued a Correction to DD Form 214, Certificate or Discharge from Active Duty (DD Form 215). Petitioner’s DD Form 215 corrects Petitioner’s social security number. f. A review on 1 July 2014 by Navy Personnel Command (NPC) PERS 312 of Petitioner’s record shows he is entitled to: Navy Unit Commendation Medal. g. Petitioner contends he is entitled to Submarine Warfare Pin and the Navy Unit Commendation Medal. CONCLUSION: Upon review and consideration of the evidence of record, the Board determined Petitioner’s request warrants relief. In light of the Navy Personnel Command (NPC) PERS 312 award review dated 25 January 2020, the Board determined Petitioner is entitled the addition of the following award not previously awarded: Navy Unit Commendation Medal. Additionally, the Board considered Petitioner’s record which reflects his entitlement to the Submarine Warfare Pin. RECOMMENDATION: In view of the above, the Board directs the following corrective action: Petitioner be issued a Correction to DD Form 214, Certificate of Release or Discharge from Active Duty (DD Form 215), to indicate his entitlement to the Navy Unit Commendation Medal, and Submarine Warfare Pin. No further changes be made to Petitioner’s record. A copy of this report of proceedings be filed in Petitioner’s naval record. 4. It is certified that a quorum was present at the Board’s review and deliberations, and that the foregoing is a true and complete record of the Board’s proceedings in the above entitled matter. 5.
