Pursuant to the provisions of reference (a), Petitioner, a former enlisted member of the U.S. Navy, filed enclosure (1) requesting an administrative change to his Armed Forces of the United States Report of Transfer of Discharge form (DD Form 214) to reflect his correct Social Security Number (SSN). Enclosures (1) and (2) apply. 2. The Board, consisting of Petitioner's allegations of error and injustice on 26 March 2021, and, pursuant to its regulations, determined that the corrective action indicated below should be taken on the available evidence of record. Documentary material considered by the Board consisted of the enclosures, relevant portions of his naval service records, and applicable statutes, regulations, policies. 3. The Board, having reviewed all the facts of record pertaining to Petitioner's allegations of error and injustice finds as follows: a. Before applying to this Board, Petitioner exhausted all administrative remedies available under existing law and regulations within the Department of the Navy. b. Although enclosure (1) was not filed in a timely manner, it is in the interests of justice to review the application on its merits. c. The Petitioner enlisted in the U.S. Navy and began a period of active service on 16 March 1964. On 15 April 1966 at the end of his active obligated service, Petitioner was separated from active duty with an honorable characterization of service. d. The SSN as reflected on Petitioner’s DD Form 214 is ” However, the Petitioner’s correct SSN is ” e. Petitioner provided a copy of his Social Security Card reflecting his correct SSN as ,”, as well as two National Agency Check Request forms (OPNAV Form 5510-397) from his service record displaying his correct SSN. CONCLUSION: Upon review and consideration of all the evidence of record, the Board concludes that Petitioner’s request warrants relief. The Board noted that the third digit of the Petitioner’s SSN as reflected on his DD Form 214 is incorrect. RECOMMENDATION: In view of the foregoing, the Board finds the existence of an error warranting the following corrective action. That Petitioner be issued a “Correction to DD Form 214, Certificate of Release or Discharge from Active Duty” (DD Form 215), to indicate that his SSN in Block 32 is changed to “ That no other changes be made to Petitioner’s record. That a copy of this report of proceedings be filed in Petitioner’s naval record. 4. It is certified that a quorum was present at the Board’s review and deliberations, and that the foregoing is a true and complete record of the Board’s proceedings in the above entitled matter. 5.
