On 31 January 1996, he elected former spouse coverage under the Survivor Benefit Plan based on full retired pay, naming XXXXXXXXXXXXXXXXXXX as former spouse beneficiary. b. The former spouse coverage was suspended effective 1 July 1996 and reinstated effective 1 October 2003.
