In support of her request, the applicant provides a copy of her AFAM/2OLC certificate, a memorandum of record (MFR) from the 802nd Fighter Support Squadron (FSS) Awards and Decorations Noncommissioned Officer in Charge (NCOIC), and a Report on Individual Personnel. The applicant’s complete submission, with attachment, is at Exhibit A. _________________________________________________________________ STATEMENT OF FACTS: Data extracted from the personnel data system reflects the applicant is currently serving in the Regular Air Force in the grade of master sergeant (E-7) with a date of rank of 1 May 2008. The remaining relevant facts pertaining to this application, extracted from the applicant’s military records, are contained in the letter prepared by the appropriate office of the Air Force at Exhibit B. _________________________________________________________________ AIR FORCE EVALUATION: AFPC/DPSIDR recommends denial. DPSIDR indicates that the MFR from the 802nd FSS Awards and Decorations NCOIC states that their decoration mail out log indicates an AFAM for the period 1 January 2007 through 4 February 2008 with Special Order #GC- 0250 was mailed to Lajes Field, Arizona on 30 January 2008; however, they are unable to produce a copy of the citation or order from their staged records. DPSIDR states that without a copy of the order they are unable to recommend granting the applicant’s request. The complete DPSIDR evaluation is at Exhibit B. _________________________________________________________________ APPLICANT'S REVIEW OF AIR FORCE EVALUATION: A copy of the Air Force evaluation was forwarded to the applicant on 15 March 2010 for review and response within 30 days (Exhibit C). As of this date, this office has received no response. _________________________________________________________________ THE BOARD CONCLUDES THAT: 1. The applicant has exhausted all remedies provided by existing law or regulations. 2. The application was timely filed. 3. Sufficient relevant evidence has been presented to demonstrate the existence of an error or injustice. Although the applicant was unable to provide a copy of a special order awarding her the AFAM w/2OLC, we believe the documentation she provided in the form of an AFAM w/2OLC citation and certificate provides a reasonable basis to conclude she was awarded the AFAM w/2OLC. Therefore, in the interest of equity and justice, we elect to resolve any doubt in favor of the applicant. Accordingly, we recommend her record be corrected as indicated below. ________________________________________________________________ THE BOARD RECOMMENDS THAT: The pertinent military records of the Department of the Air Force relating to APPLICANT be corrected to show that on 15 February 2008, she was awarded the Air Force Achievement Medal with Second Oak Leaf Cluster, for meritorious service during the period 1 January 2007 to 4 February 2008. ________________________________________________________________ The following members of the Board considered this application in Executive Session on 21 September 2010, under the provisions of AFI 36-2603: All members voted to correct the records, as recommended.
