RECORD OF PROCEEDINGS AIR FORCE BOARD FOR CORRECTION OF MILITARY RECORDS IN THE MATTER OF: DOCKET NUMBER: BC-2011-01509 COUNSEL: NONE HEARING DESIRED: NO _________________________________________________________________ APPLICANT REQUESTS THAT: His DD Form 214, Armed Forces of the United States Report of Transfer or Discharge, be corrected to reflect: 1) The Presidential Unit Citation (PUC). 2) The Air Force Outstanding Unit Award (AFOUA). _________________________________________________________________ APPLICANT CONTENDS THAT: The awards were omitted from his last DD Form 214. In support of his request, the applicant submits copies of Special Order (SO) GB-130, a Unit Awards letter, his DD Form 214 and page two of his AF Form 1712, Uniform Military Personnel Record. The applicant's complete submission, with attachments, is at Exhibit A. _________________________________________________________________ STATEMENT OF FACTS: On 30 Jun 73, the applicant was retired from the Regular Air Force. He served 20 years, 3 months and 4 days on active duty. The remaining relevant facts pertaining to this application, extracted from the applicant’s military records, are contained in the letter prepared by the appropriate office of the Air Force at Exhibit C. _________________________________________________________________ AIR FORCE EVALUATION: HQ AFPC/DPSIDR recommends denial. DPSIDR notes the applicant’s submission of SO GB-130 awarding the PUC to members of the 6234th Organizational Maintenance Squadron (OMS) with an inclusive period of 8 Nov 65 to 8 Apr 66; however, DPSIDR states the applicant was assigned to the 388th Field Maintenance Squadron (FMS) during this timeframe. The 388thFMS did not earn the PUC during his assigned timeframe. The complete DPSIDRevaluationis at Exhibit C. APPLICANT'S REVIEW OF AIR FORCE EVALUATION: The applicant again requests correction of his recordsto includethe PUC and AFOUAand provides an AF Form 1098, Personnel Action Request. The applicant’s complete submission, with attachments is at Exhibit E. THE BOARD CONCLUDES THAT: 1.The applicant has exhausted all remedies provided by existinglaw or regulations. 2.The application was not timely filed; however, it is in the interest of justice to excuse the failure to timely file. 3.Insufficient relevant evidence has been presented to demonstrate the existence of error or injustice regarding the applicant’s request for the AFOUA and PUC. Although the applicant states the AFOUA is not listed on his DD Form 214, we note that on 27 February 1991, a DD Form 215, Correction to DD Form 214,was issued to add the AFOUA with Valor and one Oak LeafCluster. Therefore, it appears this issue has been administratively resolved and there is no basis for the Board to consider this request. With respect to his request for the PUC, we note the applicant provides documentation (AF Form 1098)that reflects the6234thField Maintenance Squadron (FMS). However, the documentation he submitted is not supported by the information contained in his military personnel record. As such, we find the evidence presented insufficient to corroborate his claim that he was assigned to the 6234thFMS.We are not unmindful or unappreciative of hisservice to the Nation; however, in the absence of evidence to the contrary, we find no basis to grant the requested relief. 2 THE BOARD DETERMINES THAT: The applicant be notified that the evidence presented did not demonstrate the existence of material error or injustice; that the application was denied without a personal appearance; and that the application will only be reconsidered upon the submission of newly discovered relevant evidence not considered with this application. _________________________________________________________________ The following members of the Board considered this application in Executive Session on 7 Feb 12, under the provisions of AFI 36- 2603: The following documentary evidence was considered in AFBCMR BC- 2011-01509: Exhibit A.
