He be awarded the Iraqi Campaign Medal (ADMINISTRATIVELY CORRECTED). 2. His DD Form 214, Certificate of Release from Active Duty, be corrected to show he was awarded the Air Force Achievement Medal (AFAM). ________________________________________________________________ THE APPLICANT CONTENDS THAT: He was presented with the Iraqi Campaign Medal and the Air Force Achievement Medal, but he cannot locate the documentation for the awards. In support of his appeal, the applicant provides a copy of his DD Form 214. The applicant’s complete submission, with attachments, is at Exhibit A. ________________________________________________________________ STATEMENT OF FACTS: The applicant served in the Regular Air Force from 22 October 2002 to 9 November 2006. He was released from active duty with an honorable discharge. His DD Form 214 does not reflect he was awarded the AFAM> ________________________________________________________________ AIR FORCE EVALUATION: AFPC/DPSIDR recommends denial. They were able to locate an AF Form 638, dated 18 December 2005, in the applicant’s Master Personnel Record stating the applicant was awarded the AFAM. However, the special order needed to validate the applicant’s entitlement to wear the decoration could not be located. The complete DPSIDR evaluation is at Exhibit C. ________________________________________________________________ APPLICANT'S REVIEW OF THE AIR FORCE EVALUATION: A copy of the Air Force evaluation was forwarded to the applicant on 26 August 2011, for review and comment within 30 days. As of this date, no response has been received by this office (Exhibit D). ________________________________________________________________ THE BOARD CONCLUDES THAT: 1. The applicant has exhausted all remedies provided by existing law or regulations. 2. The application was timely filed. 3. Sufficient relevant evidence has been presented to demonstrate the existence of an error or injustice. Although the applicant was unable to provide a copy of the special order awarding him the AFAM, we believe the AF Form 638, found in his Master Personnel File provides a reasonable basis to conclude he was awarded the AFAM. Accordingly, we recommend his record be corrected as indicated below. ________________________________________________________________ THE BOARD RECOMMENDS THAT: The pertinent military records of the Department of the Air Force relating to APPLICANT be corrected to show that the DD Form 214, Certificate of Release or Discharge from Active Duty, issued in conjunction with his 9 November 2006 separation, be amended in Item 13, Decorations, Medals, Badges, Commendations, Citations and Campaign Ribbons Awarded or Authorized, to include the Air Force Achievement Medal. ________________________________________________________________ The Board considered Docket Number BC-2011-02687 in Executive Session on 4 January 2012, under the provisions of AFI 36-2603: Panel Chair Member Member All members voted to correct the record, as recommend.
