RECORD OF PROCEEDINGS AIR FORCE BOARD FOR CORRECTION OF MILITARY RECORDS IN THE MATTER OF: DOCKET NUMBER: BC-2013-00307 COUNSEL: NONE HEARING DESIRED: NO ________________________________________________________________ APPLICANT REQUESTS THAT: His 18 Dec 12 Fitness Assessment (FA) score be removed from the Air Force Fitness Management System (AFFMS). ________________________________________________________________ APPLICANT CONTENDS THAT: His FA failure was due to a previous diagnosis of anemia caused by gastrointestinal (GI) bleeding. However, due to holiday scheduling he was unable to complete the process to have his test invalidated within the allotted ten days. The applicant’s complete submission, with attachments, is at Exhibit A. _________________ ______________________________________________ STATEMENT OF FACTS: The applicant is currently serving in the Regular Air Force in the grade of technical sergeant (E-6). On 18 Dec 12, the applicant participated in the contested FA. He attained a composite score of 28.00 after receiving the following component scores: abdominal circumference – 20.00 points, push-ups – 8.00 points, sit-ups – 0.00 points, and cardio – 0.00 points, which constituted an unsatisfactory assessment. On 20 Dec 12, according to a Standard Form 600, Chronological Record of Medical Care, provided by the applicant, he informed the medical provider he was diagnosed with anemia in early November. The medical provider indicated the applicant was moderately anemic and instructed him to schedule an appointment with his Primary Care Manager (PCM) to be provided the proper documentation for his contested FA to be invalidated. On 21 Dec 12, the applicant was issued an AF Form 422, Notification of Air Force Member’s Qualification Status, which exempted him from the cardio and strength components of the FA; however, he was cleared to be tested on his height, weight, and abdominal circumference components, until 18 Feb 13. On 8 Jan 13, the applicant attained an excellent score on an FA. He was exempt from the cardio, push-up, and sit-up components of the FA. On 23 Jul 13, the applicant attained an excellent score on an FA. He was exempt from the cardio, push-up, and sit-up components of the FA. The remaining relevant facts pertaining to this application is described in the letter prepared by the Air Force office of primary responsibility (OPR) which is included at Exhibits C. ________________________________________________________________ AIR FORCE EVALUATION: AFPC/DPSIM recommends denial, noting the evidence provided by the applicant does not conclusively support his contentions. While the applicant provided evidence which indicates he was in fact seen by a medical provider and there was a discussion of an anemia diagnosis, there is no medical documentation which supports the validity of the original November 2012 anemia diagnosis. Also, it was not until after his FA failure that he became restricted from performing all components of the FA except for his abdominal circumference. Furthermore, the medical provider instructed the applicant to follow-up with his PCM to initiate the required documentation to invalidate his contested FA. While it could be assumed his medical condition contributed to an unsatisfactory performance, none of the evidence provided by the applicant substantiates such a claim. Lastly, there was no documentation provided by the applicant reflecting any attempt to follow-up with his PCM to obtain such documentation. A complete copy of the AFPC/DPSIM evaluation is at Exhibit C. ________________________________________________________________ APPLICANT'S REVIEW OF AIR FORCE EVALUATION: A copy of the Air Force evaluation was forwarded to the applicant on 18 Oct 13 for review and comment within 30 days. As of this date, no response has been received by this office (Exhibit D). ________________________________________________________________ THE BOARD CONCLUDES THAT: 1.  The applicant has exhausted all remedies provided by existing law or regulations. 2.  The application was timely filed. 3.  Sufficient relevant evidence has been presented to demonstrate the existence of an error or injustice. We took notice of the applicant’s complete submission in judging the merits of the case. While the Board notes the recommendation from AFPC/DPSIM indicating that it was not until after his FA failure that he became restricted from performing all components of the FA except for his abdominal circumference, we believe a preponderance of the evidence indicates that corrective action is warranted. In this respect, we note that the while the applicant’s exemption took place after the contested FA, the proximity of the diagnosis to the contested FA was close enough to establish there was a causal relationship between the applicant’s medical condition and his FA failure. Therefore, we recommend his records be corrected as indicated below. ________________________________________________________________ THE BOARD RECOMMENDS THAT: The pertinent military records of the Department of the Air Force relating to the APPLICANT be corrected to show that the Fitness Assessment (FA), dated 18 December 2012, be declared void and removed from the Air Force Fitness Management System (AFFMS). ________________________________________________________________ The following members of the Board considered AFBCMR Docket Number BC-2013-00307 in Executive Session on 14 Jan 14, under the provisions of AFI 36-2603: Panel Chair Member Member All members voted to correct the records as recommended.
