APPLICANT REQUESTS THAT:

The Meritorious Service Medal (MSM) be added to his DD Form 214, Certificate of Release or Discharge from Active Duty.

APPLICANT CONTENDS THAT:

He was awarded the MSM.  

In support of his request, the applicant provides copies of his MSM citation, certificate and DD Forms 214.  

The applicant’s complete submission, with attachments, is at Exhibit A.

STATEMENT OF FACTS:

On 4 Dec 1967, the applicant entered active duty.  

On 25 Oct 1987, he was discharged.

He served 19 years, 10 months and 22 days on active duty.

AIR FORCE EVALUATION:

AFPC/DPSID recommends approval for award of the MSM.  The 
applicant provided a signed certificate and citation with the 
official seal for award of the MSM with inclusive period of    
28 Jun 1980 to 31 Dec 1987.  DPSID was unable to locate a 
special order verifying award of the MSM; however, the signed 
certificate and citation appear to be authentic.  Should the 
Board approve the MSM, the applicant’s DD Form 214 will be 
amended to reflect award of the MSM.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:

On 18 Feb 2014, a copy of the Air Force evaluation was forwarded to the applicant for review and comment within 30 days (Exhibit C).  As of this date, this 
office has not received a response.

THE BOARD CONCLUDES THAT:

1.  The applicant has exhausted all remedies provided by existing law or regulations.

2.  The application was not timely filed; however, it is in the interest of justice to excuse the failure to timely file.  

3.  Sufficient relevant evidence has been presented to demonstrate the existence of an error or injustice requiring relief.  After carefully reviewing this 
application, we agree with the opinion and recommendation of the Air Force office of primary responsibility and adopt the rationale expressed that the 
applicant was awarded the MSM for outstanding service in the culmination of his career.  We note that the through date of the MSM citation and certificate 
provided by the applicant is after the applicant’s discharge; therefore, we recommend the applicant’s through date and the “Given Under My Hand” date be 
changed to reflect 25 Oct 1987.  Accordingly, we recommend the applicant’s record be corrected as indicated below.

THE BOARD RECOMMENDS THAT:

The pertinent military records of the Department of the Air Force relating to APPLICANT, be corrected to show that the DD Form 214, Certificate of Release 
or Discharge from Active Duty, issued in conjunction with his 25 October 1987 discharge, be amended in Block 13, Decorations, Medals, Badges, Citations and
Campaign Ribbons Awarded or Authorized, to reflect the Meritorious Service Medal with a through date and “Given Under My Hand” date of 25 October 1987.
