2. The applicant requests that his DD Form 214 (Report of Separation) for the period 29 July 1987 to 26 July 1991 be corrected to reflect all of his awards to include those from Desert Storm. 3. Available records show the applicant was entitled to and awarded the following awards and decorations that are not shown on the DD Form 214 issued to him on 26 July 1991: The Meritorious Unit Commendation for the period 12 September 1990 through 8 April 1991. Combat Infantryman Badge for the period 25 February 1991 through 28 February 1991. The Army Achievement Medal for the period 3 December 1990 through 15 December 1990 (2d award). The Army Commendation Medal for the period 24 August 1989 through 23 August 1990 (3d award). The Army Achievement Medal for the period 16 May 1989 through 8 July 1989 (3d award). Good Conduct Medal for the period 22 July 1987 through 21 July 1990 (2d award). 4. Army Regulation 635-5 establishes policy and outlines procedures for the preparation of DD Form 214. It states, in pertinent part, that entries for decorations, medals, badges, citations and campaign ribbons awarded or authorized will be for all periods of service. CONCLUSIONS: 1. An administrative error was made in the preparation of the DD Form 214 effective 26 July 1991 in that not all of the applicant’s awards were shown. 2. In view of the foregoing, it would be appropriate to correct the applicant’s records as indicated below. RECOMMENDATION: That all of the Department of the Army records related to this case be corrected by changing the applicant’s DD Form 214, to show all of the aforementioned awards.
