Exhibit B - Military Personnel Records (including advisory opinion, if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, that his DD Form 214 (Certificate of Release or Discharge From Active Duty) that was issued at the time of his release from active duty on 15 March 2006 be corrected to show that he was awarded the Bronze Star Medal and the Combat Infantryman Badge and that his correct primary military occupational specialty (PMOS) is 11B2V. 2. The applicant essentially states that this DD Form 214 was filled out in February 2006, and he did not receive his orders for the Bronze Star Medal and the Combat Infantryman Badge until the second week of March 2006. He also states that upon graduation from the Ranger Course in 1999, he was awarded PMOS 11B2V, but that his DD Form 214 which was issued at the time of his release from active duty on 15 March 2006 shows that his PMOS was 11B2O. 3. The applicant provides his DD Form 214 that was issued to him at the time of his release from active duty on 15 March 2006, orders and a certificate which award him the Bronze Star Medal, and orders awarding him the Combat Infantryman Badge in support of this application. He also provides his DD Form 214, orders awarding him the Ranger Tab and PMOS 11B2V, and deployment orders in support of this application. CONSIDERATION OF EVIDENCE: 1. The applicant is requesting correction of an alleged error which occurred on 15 March 2006, the date of his release from active duty. The three applications submitted in this case are dated 15 March 2006. 2. Item 11 (Primary Specialty) of the applicant’s DD Form 214 which was issued at the time of his release from active duty on 15 March 2006 shows that his PMOS was 11B2O (Infantryman). The fifth character of his PMOS, the Special Qualification Identifier (SQI), has an entry of “O,” which signifies that the applicant had no special qualifications. However, he provided orders which show that he was awarded PMOS 11B2V on 22 December 1998 as a result of successfully completing Ranger training. The SQI “V” signifies that the applicant is a Ranger Parachutist. His official military personnel file (OMPF) contains a diploma which shows that he graduated from the Ranger Course at Fort Benning, Georgia on 29 January 1999. It also shows that he completed the Airborne Course in June 1991. 3. Item 13 (Decorations, Medals, Badges, Commendations, Citations, and Campaign Ribbons Awarded or Authorized) of the applicant’s DD Form 214 which was issued to him at the time of his release from active duty on 15 March 2006 does not show that he was awarded the Bronze Star Medal or the Combat Infantryman Badge. However, he was awarded the Bronze Star Medal by Headquarters, Combined Forces Command – Afghanistan Permanent Orders Number 063-016, dated 4 March 2006. He was also awarded the Combat Infantryman Badge by Headquarters, Combined Forces Command – Afghanistan Permanent Orders Number 072-003, dated 13 March 2006. DISCUSSION AND CONCLUSIONS: 1. The applicant contends that his DD Form 214 that was issued at the time of his release from active duty on 15 March 2006 should be corrected to show that he was awarded the Bronze Star Medal and the Combat Infantryman Badge. 2. Permanent orders awarded the applicant the Bronze Star Medal and the Combat Infantryman Badge prior to the applicant’s release from active duty on 15 March 2006, which are not shown on his DD Form 214. Therefore, he is entitled to correction of his DD Form 214 that was issued at the time of his release from active duty on 15 March 2006 to show that he was awarded the Bronze Star Medal and the Combat Infantryman Badge. 3. Additionally, the evidence of record shows that the applicant graduated from both the Airborne Course and the Ranger Course, and that he was awarded PMOS 11B2V. Therefore, he is entitled to correction of his military records to show that his PMOS at the time of his release from active duty on 15 March 2006 was 11B2V.
