THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, that the Army Commendation Medal (ARCOM) and two Army Achievement Medals (AAM) be added to his DD Form 214 (Certificate of Release or Discharge From Active Duty) that was issued to him at the time of his release from active duty on 27 November 2005. 2. The applicant essentially states that these awards are missing on his DD Form 214 that was issued to him at the time of his release from active duty on 27 November 2005. 3. The applicant provides his DD Form 214 that was issued to him at the time of his release from active duty on 27 November 2005, and a DA Form 638 (Recommendation for Award) and certificate awarding him the ARCOM in support of this application. CONSIDERATION OF EVIDENCE: 1. The applicant is requesting correction of an alleged error which occurred on 27 November 2005, the date of his release from active duty. The application submitted in this case is dated 15 June 2006. 2. Neither Item 13 (Decorations, Medals, Badges, Commendations, Citations, and Campaign Ribbons Awarded or Authorized) nor Item 18 (Remarks) of his DD Form 214 that was issued to him at the time of his release from active duty on 27 November 2005 shows that he was awarded the ARCOM. However, he was awarded the ARCOM by Headquarters, Division Artillery, 42nd Infantry Division (Mechanized) Permanent Orders Number 215-4, dated 5 August 2005. 3. The applicant requested that his DD Form 214 be corrected to show two awards of the AAM; however, the first entry in Item 13 of his DD Form 214 reads, “Army Achievement Medal (2nd Award).” DISCUSSION AND CONCLUSIONS: 1. The applicant contends that the ARCOM and two AAMs should be added to his DD Form 214 that was issued to him at the time of his release from active duty on 27 November 2005. 2. Permanent orders awarded the applicant the ARCOM which is not shown on his DD Form 214 that was issued to him at the time of his release from active duty on 27 November 2005. Therefore, he is entitled to correction of his military records to show the award of the “Army Commendation Medal.” 3. As the applicant’s DD Form 214 that was issued to him at the time of his release from active duty on 27 November 2005 already shows that he was awarded the “AAM (2nd Award),” there is no basis for granting relief to this portion of the applicant’s request. The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by showing the award of the Army Commendation Medal. 2. The Board further determined that the evidence presented is insufficient to warrant a portion of the requested relief.
