Exhibit B - Military Personnel Records (including advisory opinion, if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests that item 12a (Record of Service Date Entered AD This Period) of her DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected. 2. The applicant states, in effect, item 12a of her DD form 214 shows that her entry date on active duty is 6 May 2006. However, she entered the military under the Delayed Entry Program on 2 February 2005 and entered active duty on   4 August 2005. 3. The applicant provides a copy of her DD Form 4/1 (Enlistment/Reenlistment Document) dated 1 January 2001, and DD Form 214. CONSIDERATION OF EVIDENCE: 1. The applicant's military service records and DD Form 4 (Enlistment/ Reenlistment Document) shows that she entered active duty on 4 August 2005. She completed the necessary training and was awarded the military occupational specialty (MOS) 68W (Health Care Specialist). 2. The applicant's last duty assignment was at Walter Reed Army Medical Center Student Company in Washington, District of Columbia. The applicant received a general discharge on 30 May 2007. 3. Item 12a of her DD Form 214 shows the entry 2006 05 06. Item 12c (Net Active Service This period) of her DD Form 214 shows the entry 1 year and 25 days. 4. A research of the applicant's Official Military Personnel File (OMPF) shows that the applicant enlisted in the Regular Component of the Armed Forces on  4 August 2005. 5. Army Regulation 635-5 (Separation Documents) paragraph 2-4h(12)(a) states, in pertinent part, enter the beginning date of the continuous period of active duty for which a DD Form 214 was not previously issued. Also, enter the amount of service during the period covered by the DD Form 214 in item 12c, by subtracting item 12a Record of Service Date Entered AD This Period from item 12b Record of Service Separation Date This period. Any lost time under 10, USC 972 and non-creditable time after Expiration Term of Service (ETS), if any, are deducted. Such time will be entered in item 18 (Remarks) of the DD Form 214. If the Soldier is released from active duty because of voided enlistment, enter "00 00 00." DISCUSSION AND CONCLUSIONS: 1. The applicant requests that item 12a of her DD Form 214 be corrected to show 4 August 2005. 2. The evidence shows that the applicant's Date Entered AD This Period was  4 August 2005. Therefore, the applicant is entitled to correction of her records to show in item 12a, Date Entered AD This Period 2005 08 04. 3. Based on this correction, item 12c, Net Active Service This Period should be corrected to show 1 year, 9 months, and 27 days.
